All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. OBSERVED SLIGHT LIME BUILD UP INSIDE ICE MACHINE AND FOOD DEBRIS INSIDE PREP AREA REACH IN COOLER DOOR RUBBER GASKETS. MUST CLEAN AND MAINTAIN ALL UNCLEAN EQUIPMENT.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. MUST KEEP FLOOR DRY AT 2ND FLOOR SERVICE STATION UNDER 3-COMPARTMENT SINK.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. PREP AREA 3-COMPARTMENT SINK COLD WATER HANDLES LEAKING AND 2ND FLOOR 3-COMPARTMENT SINK PIPE LEAKING UNDER RIGHT SIDE COMPARTMENT. MUST REPAIR AND MAINTAIN SINK.

LIGHT SHIELD MISSING IN WALK-IN FREEZER-MUST REPLACE. Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities.
CAULKING ALONG THE SPLASH GUARDS AT DISHROOM AREA (SINKS & DISHMACHINE), MUST BE REPLACED, DUE TO EVIDENCE OF MILDEW. HEAVY ICE BUILD-UP IN THE CHEST FREEZER (BASEMENT) MUST BE REMOVED & MAINTAINED. All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair.
ANY OPENINGS ON CEILING AT REAR OF BASEMENT STORAGE AREA MUST BE SEALED. ALL WATER-STAINED CEILING TILES BY THE BASEMENT WALK-IN COOLER AREA MUST BE REPLACED. The walls and ceilings shall be in good repair and easily cleaned.
FLOORS UNDER STORAGE SHELVINGS AT THE FRONT DRY STORAGE ROOMS OF THE BASEMENT MUST BE KEPT CLEAN & FREE OF DEBRIS/DUST/DIRT. The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair.
