FRONT DOOR NOT PROTECTED AND REAR DOOR LEADING TO THE 2ND FLOOR APPTS. INSTRUCTED OWNER TO MAKE OUTERS TIGHT FITTING/RODENT-INSECT PROOF.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. RAW SURFACES MUST BE SEALED WITH A NON-TOXIC PAINT OR SEALANT, i.e. SHELVES IN RESTAURANT AREA, CUBBIES IN PRODUCE AREAS etc.
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. DETAIL / PAINT CLEAN ALL EQUIPTMENT THROUGHOUT i.e.stoves, coolers including all gaskets on coolers, shelves, deep fryers, mop sink.

THE PLUMBING STACK IN THE REAR PREP AREA HAS A MISSING CAP AND IS TAPED CLOSED, INSTALL A WATER TIGHT CAP SO THE PIPE IS PROPERLY SEALED.
KNIVES AND UTENSILS WITH FOOD RESIDUE ARE STORED IN A DIRTY HOLDER, REMOVE KNIVES AND UTENSILS AND WASH, RINSE, SANITIZE AND STORE THEM IN A SANITARY AND EASY TO CLEAN PLACES.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. THE FLOORS ARE DIRTY UNDER KITCHEN SHELVES AND THE PALLETS IN BOTH WALK-IN COOLERS. CLEAN AND MAINTAIN THE FLOORS AND BASEBOARDS SO THEY ARE SMOOTH AND SANITARY FROM CORNER TO CORNER.

OVER 20 LIVE MOTHS AND GRAIN BEETLES WERE FOUND IN THE FLOUR AND SUGAR STORAGE AREA SHELVING AND ON THE OUTSIDE PACKAGING. MOTHS WERE ALSO FOUND ON THE PAPER STREAMERS ABOVE THE PRODUCE SHELVING. OVER 10 SMALL FLIES WERE FOUND AROUND THE GARBAGE CONTAINER IN THE REAR COOLER AREA. DISASSEMBLE THE FLOUR STORAGE SHELVES, CLEAN, SANITIZE AND REMOVE ALL EVIDENCE OF INSECT ACTIVITY. REAR GARAGE DOOR NEEDS HAS A QUARTER INCH GAP.INSTRUCTED TO REPAIR DOOR SO AS TO BE RODENT PROOFED.
CUTTING BOARDS,SHELVING AND CHEST FREEZER NEED CLEANING. REMOVE OLD THREE COMPARTMENT SINK USED AS A STORAGE SHELVING IN THE WALK IN COOLER. WASH,RINSE AND SANITIZE ABOVE.
FLOORS AT THE NORTH WALK IN COOLER NEED CLEANING AND HAVE A DIRTY CHANNEL CUT INTO THE CONCRETE WHICH NEEDS TO BE REPAIRED SO AS BE SMOOTH AND EASILY CLEANABLE.
WALLS AND CEILINGS NEED CLEANING AT REAR STORAGE ,DISWASHING AND SEATING AREA NEED CLEANING AND OR PAINTING.
HOODS AND FILTERS ABOVE HOT LINE AREA NEED CLEANING. THE BUTCHER THREE COMPARTMENT SINK HAS DAMAGED STOPPERS, OBTAIN NEW STOPPERS AND CONTINUE TO WASH, RINSE AND SANITIZE IN THE DISHROOM UNTIL NEW STOPPERS ARE OBTAINED.
