The walls and ceilings shall be in good repair and easily cleaned. FOUND HEAVY WATER STAIN AND PEELING PAINT ON WALLS (HALLWAY-3RD FLOOR) ACROSS ROOM 304 AND HEAVY PEELING PAINT ON WALLS AND CEILING ON LANDING BETWEEN 2ND AND 3RD FLOOR. INSTRUCTED TO FIX. ----- MUST REPLACE WATER STAINED CEILING TILES ABOVE SERVING LINE AND MISSING CEILING TILES IN PAPER GOODS STORAGE ROOM.

Evidence of rodent activity found on premises. Found 5 mice droppings behind deep freezer and 3 in corner behind storage rack, and approximately 10 mice dropping under last serving line. Also, exterior doors not rodent proofed, found south west door in middle with 1/4 inch gap, west door in middle with 1/4 inch gap and north door in middle with 1/4 inch gap. Pest control logbook not maintained or provided during inspection. Instructed to remove all rodent activity, detail clean areas, rodent proof all doors and have licensed pest control company service premises and provide logbook with all written documentation. Serious citation issued 7-38-020.
