A separate and distinct offense shall be deemed to have been committed for each Minor violation that is not corrected upon re-inspection by the health authority. Previous minors from 6/18/09 visit (report # 159887) not corrected at this time. (#34-soda syrup boxes must be elevated off the floors under soda machine; #38-must fix/provide ventilation in bathroom; #40-internal thermometers must be provided at all coolers thruout store). Citation issued.

The walls and ceilings shall be in good repair and easily cleaned. WALL PANELS LOOSE FROM WALL DUE TO DAMAGED WALL CORNER SEAL, BY MOP SINK. MUST REATTACH DAMAGED WALL PANELS TO WALL.

TOILETBOWL HANDLE LOOSE. MUST REPAIR AND MAINTAIN HANDLE.

THE RIGHT HINGE OF THE ICE MACHINE LID IS IN POOR REPAIR, MUST FIX.
UNNECESSARY ARTICLES IN DRY STORAGE ROOM MUST BE REMOVED, BETTER ORGANIZED & ELEVATED OFF THE FLOOR-IF NEEDED.
LEAKY FAUCET OF THE 3-COMP SINK MUST BE FIXED.
