All dishwashing machines must be of a type that complies with all requirements of the plumbing section of the Municipal Code of Chicago and Rules and Regulation of the Board of Health. Observed water with food debris backing up in middle compartment of 3 compartment sink in prep area from grease trap on right side of sink. Water was draining when flowing from faucet. Instructed manager to have sink maintained. citation #57995-10
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. Must provide back flow protection device for faucet in boiler room ensure no hose use.
All rooms in which food or drink is prepared, or in which utensils are washed, shall be lighted so that a minimum of 50-foot candles of light is available on all work surfaces. Provide shatterproof lighting above dining area.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. Must remove duct tape from chest freezer in food prep area. Repair if needed.
The walls and ceilings shall be in good repair and easily cleaned. Must remove milk crates and provide proper shelving for food prep area and food storage area. Store items 6' off floor.

Floors in kitchen areas, teacher lounge areas need cleaning, floors in eating areas(hallways)need cleaning.Floor in bsmt. storage areas, bathrooms need cleaning.
Peeling paint on walls, ceilings in classrooms#'s 302,303, shall be repair. Hallway ceiling tiles by elevator that were damaged, stained, shall be replaced. Classrooms in 2nd floor, and 1st floor that had peeling paint, exposed plaster, shall be repair. bathrooms that had peeling paint, ceiling gaps shall be repair(1st,2nd, and 3rd floors).Exposed plaster in kitchen by vent, shall be repair.
Damaged, broken, missing light shields in kitchen prep area, teachers lounge, classrooms, hallways, and bathrooms shall be replacec(1st,2nd,and 3rd floors)shall be replaced. Light shields in prep area, bathrooms, classrooms, hallways not clean need cleaning.
Ehaust canopy not clean needs cleaning, exhaust vents(ventilation)in coolers need cleaning.

WASTE WATER BACKING UP ON THE 3RD COMPT SINK OF THE 3 COMPT SINK. INSTRUCTED MANAGER TO REPAIR AND STOP USING THE 3 COMPT SINK.CITATION ISSUED SERIOUS. FIELD SUPERVISOR T.S.NOTIFIED. (PIPE UNDER THE 3 COMPT SINK IS SEPARATED . (FIRST COMPT HAS EACH OWN PIPE AND GREASE TRAP BOX. THE 2ND AND 3RD COMPT SINK HAS EACH OWN PIPE CONNECTING TO THE ANOTHER GREASE TRAP BOX).PLUMBER CAME AND FIXED THE PROBLEM WHILE I WAS TYPING THE REPORT.THE 3RD COMPT SINK IS NOW DRAINING WELL. INSTRUCTED THE MANAGER TO SANITIZE THE SINK BEFORE USING.
FOUND NO HOT WATER ON THE 3RD FLOOR BOYS AND GIRLS WASHROOM, 2ND FLOOR BOYS WASHROOM, 1ST FLOOR GIRLS AND BOYS WASHROOM, BASEMENT TEACHERS LOUNGE ROOM. INSTRUCTED MANAGER THAT ALL WASHROOM HAS TO HAVE HOT RUNNING WATER UNDER CITY PRESSURE. ENGINEER CAME AND WAS ABLE TO PROVIDE HOT RUNNING WATER WHILE I WAS TYPING THE REPORT.CITATION ISSUED SERIOUS. FIELD SUPERVISOR T.S. NOTIFIED.

Ceiling ventilation cover inside both washroom in the Mobile school. Must clean and maintain. Damaged wall base on the 2nd floor landing by the stair. Must repair and maintain. Room 301 classroom with peeling paint on the ceiling. Must scrape the peeling paint and maintain. Peeling paint on the walls inside the principal washroom. Must scrape peeling paint and maintain.
