All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. BAR GUN HOLSTERS NOT CLEAN, SLIGHT BUILD UP. MUST CLEAN AND SANITIZE HOLSTERS.
The walls and ceilings shall be in good repair and easily cleaned. OBSERVED DUST BUILD UP ON PREP AREA CEILING TILES AROUND VENT COVERS. MUST CLEAN AND MAINTAIN ALL UNCLEAN CEILING TILES.
All parts of the food establishment and all parts of the property used in connection with the operation of the establishment shall be kept neat and clean and should not produce any offensive odors. REAR STORAGE ROOM CLUTTERED. MUST ELEVATE ALL ITEMS 6' OFF FLOOR TO PREVENT POTENTAIL PEST HARBORAGE ALSO MUST REMOVE ALL PLASTIC MILK CRATES AT SAME LOCATION.1

ALL CUTTING BOARDS WITH DEEP/DARK GROOVES MUST BE REPLACED. All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair.
INTERIOR OF EXHAUST VENTS AT BOTH ENDS OF THE DISHMACHINE, WALK-IN COOLERS SHELVES & SOME FAN GUARDS OF COOLERS MUST BE FREE OF DUST BUILD-UP/DIRT/FOOD DEBRIS. All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily.
FLOORS AT DISHROOM & AT THE WAIT STAFF STATION BY THE BAR MUST BE KEPT DRY TO PREVENT PEST HARBORAGE. THE METAL PLATE IN REAR WALK-IN COOLER IS LOOSE-MUST FIX & SEAL, TO PREVENT DEBRIS FROM ACCUMULATING. The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair.
A WATER LEAK OBSERVED FROM ONE OF THE HOSES UNDER THE DISHMACHINE & FAUCET WASHERS AT 1-COMP SINKS IN KITCHEN MUST BE FIXED. Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health.
ALL CLEANING EQUIPMENTS (BROOMS/MOPS) MUST STORED CLEAN & HANGING ON WALL MOUNTS (NOT DIRECTLY ON THE MOP ROOM FLOOR). Cleaning equipment shall be properly stored away from food, utensils, equipment, and clean linens.
