All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. FOUND 30-35 MICE DROPPINGS IN THE ELECTRIC/STORE ROOM AND BOILER ROOM. NO PEST CONTROL RECIEPT OR LOG BOOK ON SITE. THE EAST DOUBLE DOORS BY THE LUNCHROOM AND THE SOUTH DOOR HAS A ONE FOURTH GAP AT THE BOTTOM OF THE DOORS NOT TIGHT FITTING OR RODENT PROOF. SERIOUS CITATION GIVEN 7-38-020. INSTRUCTED TO CLEAN UP THE AREAS AND CALL A PEST CONTROL COMPANY TO COME HERE AND SREVICE THIS ESTABLISHMENT. ALSO PROVIDE A COMPLETE LOG BOOK. RECEIPTS, MSDS -MATERAL SAFETY DATA SHEETS, A MAP WHERE THE BAIT BOXES ARE LOCATED UPON MY NEXT INSPECTION.

The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. CLEAN THE FLOORS IN THE BASEMENT. PROVIDE FLOOR TILES IN THE GIRLS WASHROOM.
None of the operations connected with food establishments shall be conducted in any room used as living or sleeping quarters. TWO OF THE STALL DOORS IN THE GIRLS WASHROOM ARE IN POOR REPAIR WILL NOT SHUT PROPRELY OR STAY CLOSED. INSTRUCTED TO REPAIR OR REPLACE SAME.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. REPLACE THE MISSING VENT IN THE GIRLS WASHROOM.
