Containers and utensils shall be inverted, covered, or otherwise protected from contamination until used. All single-service drinking straws and containers shall be discarded immediately after use. CLEAN MULTI-USE UTENSILS MUST BE PROPERLY STORED ON STORAGE SHELVES IN KITCHEN.
All food not stored in the original container shall be stored in properly labeled containers. MUST LABEL DRIED FOOD STORAGE CONTAINERS IN KITCHEN
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. MUST SEAL RUSTY STORAGE SHELVES AT DISH WASHING AREA AND REPLACE BROKEN GASKETS ON TWO DOORS STORAGE COOLER IN DISH WASHING AREA.
All utensils shall be thoroughly cleaned and sanitized after each usage. THE FOLLOWING NOT CLEAN; MUST CLEAN/SANITIZE; COOLERS, CHEST FREEZERS, EXTERIOR OF ICE MACHINE, MEAT SLICER, COOKING EQUIPMENT, DRIED FOOD STORAGE CONTAINERS, STORAGE SHELVES UNDER FRONT SERVING COUNTER, MOP SINK AND CABINETS NEAR FRONT SERVING AREA.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. MUST REPLACE BROKEN FLOOR TILES THRU-OUT PREMISES AND CLEAN FLOORS.
The walls and ceilings shall be in good repair and easily cleaned. MUST REPAIR WALLS AND CEILING AT REAR DISH WASHING AREA, AND REPLACE MISSING AND DAMAGED CEILING PANELS AND SEAL WALL OPENINGS INSIDE STORAGE AND FURNACE ROOMS TO PREVENT PEST ENTRY.
Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities. MUST REPLACE LIGHT SHIELDS WHERE NEEDED AND PROVIDE ADEQUATE LIGHTING IN FURNACE ROOM.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. MUST REPAIR LEAKING FAUCET FOR 3PART SINK AND LEAKING PIPES UNDER SAME SINK; ALSO MUST SEAL RUSTY GREASE TRAP UNDER SAME SINK.
EXCESSIVE CLUTTER IN STORAGE AND FURNACE ROOM, MUST REMOVE UNNECESSARY ARTICLES, CLEAN AND ORGANIZE TO PREVENT PEST HARBORAGE. ARTICLES MUST BE STORED OFF FLOORS, AWAY FROM WALLS.
