MUST REPAIR LEAKING SEAL AT 3 COMPARTMENT SINK
All parts of the food establishment and all parts of the property used in connection with the operation of the establishment shall be kept neat and clean and should not produce any offensive odors. MUST CLEAN UTILITY CLOSET, REMOVE ALL CLUTTER

NO RUNNING HOT WATER IN EMPLOYEES BATHROOMS AT HAND SINK, INSTRUCTED TO PROVIDE HOT WATER AT ALL TIMES, CRITICAL CITATION ISSUED ENGINEER MADE ADJUSTMENTS TO WATER TANK AND HOT WATER WAS PROVIDE, INSTRUCTED TO HAVE HOT WATER AT ALL TIMES
When toilet and lavatory facilities are provided for the patrons of food establishments, such facilities shall be adequate in number, convenient, accessible, properly designed, and installed according to the municipal code. NO RUNNING HOT WATER IN BOYS AND GIRLS BATHROOM AT HAND SINKS, MUST PROVIDE AT ALL TIMES CRITICAL CITATION ISSUED ENGINEER MADE ADJUSTMENTS TO WATER TANK AND HOT WATER WAS PROVIDE, INSTRUCTED TO HAVE HOT WATER AT ALL TIMES

The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. REPAIR MISSING/DAMAGED FLOOR TILES IN BOTH BOYS AND GIRLS WASH ROOMS. REPLACE ALL MISSING COVING THROUGHOUT.
The walls and ceilings shall be in good repair and easily cleaned. WALLS IN BOYS WASHROOM AND IN KITCHEN AREA NEAR THE EXPOSED HAND SINK NOT MAINTAINED,i.e.KITCHEN AREA LOOSE PAINT, BOYS WASHROOM BEHIND THE URINALS LOOSE PAINT/RUSTY. REPAIR SAME. REPLACE DAMAGED AND WATER STAINED CEILING TILES.

IN THE BOYS TOILET ROOM 1ST FLOOR THE FOURTH URINAL IS BROKEN. INSTRUCTED TO REPAIR OR REPLACE IT.
PEPLACE THE FLOOR TILE UNDER THE 3 COMPARTMENT IN PREP AREA STAINED LOOSE AND SOME BROKEN.
CLEAN THE VENTS IN THE WALLS IN BOYS AND GIRLS TOILET ROOMS THEY HAVE DUST BUILDUP ON THEM.
