All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. Must sand/bleach or replace cutting boards with deep/dark grooves.
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. Following need cleaning: interior floors of coolers, interior bottom of fryers, due to food debris/stains.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. Floors in storage room & rear hallway must be kept clean; all stock must be elevated off the floor.
Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities. Must replace missing light shield in rear of prep area.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. Must fix leaky faucet at 3-comp sink.

The area outside of the establishment used for the storage of garbage shall be clean at all times and shall not constitute a nuisance. A FOOT OF GARBAGE IS PILED UP UNDER THE DUMPSTERS AND THE OUTSIDE OF THE GREASE CONTAINER IS FULL OF GREASE BUILDUP IN THE ALLEY USED BY THE ABOVE, REMOVE ALL SPILLED WASTE AND MAINTAIN THE DUMPSTER AND GREASE CONTAINER FREE OF SPILLED WASTE AND GREASE.
A separate and distinct offense shall be deemed to have been committed for each Minor violation that is not corrected upon re-inspection by the health authority. The previous minor violations were originally documented on 5/21/10, report # 229426 and include: (32) Must sand/bleach or replace cutting boards with deep/dark grooves. (33) Following need cleaning: interior floors of coolers, interior bottom of fryers, due to food debris/stains. (34) Floors in storage room & rear hallway must be kept clean; all stock must be elevated off the floor. (36) Must replace missing light shield in rear of prep area.
THE 3-COMPARTMENT SINK INCLUDING THE SANITIZING COMPARTMENT HAS BLACK, GREASY BUILDUP IN THE CORNERS OF THE SINK INTERIOR AND BACK SPLASH, WASH, RINSE AND SANITIZE ALL OF THE FOOD DISPENSING UTENSILS AND EQUIPMENT UNTIL IT IS CLEAN, SANITARY AND MAINTAINED ON A DAILY BASIS.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. ALL FOOD AND NON-FOOD EQUIPMENT IS GREASY AND DIRTY; WASH, RINSE AND SANITIZE ALL OF THE FOOD DISPENSING UTENSILS AND EQUIPMENT UNTIL IT IS CLEAN, SANITARY AND MAINTAINED ON A DAILY BASIS.
CLEAN ALL DRIPPING GREASE FROM THE EXHAUST HOOD.
THE FLOORS KITCHEN AND UPSTAIRS STORAGE AREA FLOORS ARE FILTHY, THE FLOORS ARE DIRTY THROUGHOUT THE FACILITY. CLEAN AND MAINTAIN THE FLOORS AND BASEBOARDS SO THEY ARE SMOOTH AND SANITARY FROM CORNER TO CORNER.
The walls and ceilings shall be in good repair and easily cleaned. THE WALLS THROUGHOUT THE KITCHEN ARE FILTHY; CLEAN AND MAINTAIN THE WALLS AND CEILINGS SO THEY ARE SMOOTH AND SANITARY FROM TOP TO BOTTOM.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. CAP OFF THE OPEN DRAIN LINES IN THE BATHROOM AND KITCHEN.
All parts of the food establishment and all parts of the property used in connection with the operation of the establishment shall be kept neat and clean and should not produce any offensive odors. CLEAN AND REORGANIZE THE UPSTAIRS STORAGE AREA, REMOVE ALL PILED UP DEBRIS AND EQUIPMENT.

THE FLOORS ARE NOW CLEAN.
THE WALLS ARE NOW CLEAN.
THE OPEN DRAIN LINES IN THE BATHROOM AND KITCHEN ARE NOW CAPPED.
THE UPSTAIRS STORAGE AREA HAS BEEN REORGANIZED.
WIPING CLOTHS ARE NOW STORED IN SANITIZING SOLUTION IN BETWEEN USE.
