The walls and ceilings shall be in good repair and easily cleaned. NOTED PEELING PAINT ON CEILING AT ENTRANCE #3. INSTRUCTED TO REMOVE.
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. INSTRUCTED TO CLEAN AND MAINTAIN INTERIOR OF MICROWAVE OVEN IN TEACHERS LOUNGE. NOTED DRIED FOOD DEBRIS ACCUMULATION.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. INSTRUCTED TO REPLACE MISSING FAUCET AT ONE OF 4 HANDSINK IN BOYS WASHROOM IN ANNEX.

All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. INSTRUCTED TO FIX OR REPLACE BROKEN RUBBER GASKET AROUND REACH-IN MIDDLE DOOR IN THE THE KITCHEN.

Toilet room facilities not maintained for students. Observed several students in annex building going and coming from toilet room without proper hand washing. Inside toilet rooms not provided with liquid soap or paper towel. Engineer stated that teachers provided soap and paper towel for students. However, both student were without soap or paper towel before and after entering toilet room. Also, no written policy is available or posted in regards to hand washing procedure. Instructed to have written policy available and/or posted and to monitor children for proper hand washing. Serious citation issued 7-38-030.
Floor in kitchen not maintained. Instructed to replace or repair broken floor tile.
Ceiling tile missing in janitors closet. Instructed to replace missing ceiling tile. Wall and baseboard in church additional building not maintained. Instructed to remove peeling and chipped paint and repair and repaint walls. Door frame near east entrance not maintained. Instructed to seal gaps.
Thermometer not provided in classroom(mobile) cooler. Instructed provide thermometer visible inside cooler.
Commercial table top convection oven inside classroom(mobile). Instructed to remove commercial equipment or have it properly vented.
Exhaust fan not working in mobile unit toilet room. Instructed to repair exhaust fan.

OBSERVED OVERHEAD DOOR AT DOCK AREA NOT SEAL AND SOME ENTRANCE AREA DOORS NOT RODENT PROOF, APPROX., 1/4 GAP OPENING AT BOTTOM OF DOORS MENTION, INSTRUCTED TO SEAL, SERIOUS CITATION ISSUED: 7-38-020
OBSERVED NO SOAP FOR HAND WASHING IN CHILDREN'S BATHROOM IN THE ANNEX BUILDING, SIGNS ON WALL STATES THAT SOAP IS SUPPLIED BY TEACHERS, HOWEVER SAW 3 CHILDREN GOING INTO WASHROOM WITHOUT SOAP IN HAND TO PROPERLY WASH HANDS, MUST PROVIDE SOAP AT ALL TIMES, CRITICAL CITATION ISSUED: 7-38-030
MUST STORE PERSONAL ITEM IN ONE DESIGNATED AREA NICE AND NEATLY
MUST REMOVE DUST BUILDUP AT VENTS ABOVE 3 COMPARTMENT SINK AND NEXT TO REHEATING UNITS, IN KITCHEN AREA
