Dishwashing facilities not maintained or operated. While draining water in left of three compartment sink, water back flowing to right side. Water backflowing is yellow grease and food particles. Instructed to detail clean grease trap and drain pipes to prevent waste water not properly disposing. Serious citation issued 7-38-030.
Light bulbs not properly shielded in prep area. Instructed to shield bulb to prevents bulbs shattering.
Floor along wallbase in prep area and in boiler room not cleaned. Instructed to detail clean.
Coving tile missing on wall near door in prep area. Instructed to replace missing tile.
Ice machine panel, sink and storage shelves under prep table not cleaned. Instructed to deatil clean and sanitize.

Floors in kitchen prep area under 3 compartment sink, grease trap not clean, need detailed cleaning(corners), floors under storage shelving not clean, need cleaning.
Cove moulding behind 3 compartment sink, exposed hand sink in kitchen prep area in state of disrepair, shall be repaired/replaced, wall under dish machine area had peeling paint, shall be repaired/sealed.
Light shield metal casings that had peeling paint, rust in kitchen prep area, shall be repaired/sealed, light shields in prep area need cleaning, missing light shields in food storage room shall be provided.
