OBSERVED OVER 30 LIVE SMALL FLIES AT BAR, KITCHEN AND DISHWASH AREA. MUST REMOVE ALL INSECT ACTIVITY. ADVISED TO CALL PEST CONTROL OPERATOR FOR SERVICE. CITATION ISSUED. TICKET #H000062264 13.

All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. MUST REPLACE/REPAIR ALL DAMAGED COOLER DOOR RUBBER GASKETS IN KITCHEN.

OBSERVED 0-PPM CHLORINE SANITIZER AT LOW-TEMP DISHMACHINE. CHEMICAL SANITIZING DISHMACHINE MUST HAVE AT LEAST 100PPM-CHLORINE LEVEL AT ALL TIMES. INADEQUATE SANITIZING OF EQUIPMENT/UTENSILS (7-38-030). Equipment and utensils should get proper exposure to the sanitizing solution during the rinse cycle. Bactericidal treatment shall consist of exposure of all dish and utensil surfaces to a rinse of clean water at a temperature of not less than 180F.
CUTTING BOARDS WITH DEE GROOVES MUST BE REPLACED. All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair.
MOST WALK-IN COOLER SHELVES, CABINETS UNDER WAIT STAFF STATION, EXTERIOR OF BULK FOOD CONTAINERS IN WALK-IN COOLER, NEED CLEANING DUE TO FOOD STAINS/SPILLS. All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily.
FLOORS UNDER WALK-IN COOLER SHELVINGS MUST BE KEPT CLEAN. ALL STOCK/SUPPLIES MUST BE ELEVATED OFF THE FLOOR. FLOORS IN DISHROOM MUST BE KEPT DRY. The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair.

WATER LEAK UNDER DISHMACHINE MUST BE FIXED, TO PREVENT PEST HARBORAGE. MISSING COVERS FOR DRY FOOD LARGE CONTAINERS. INSTRUCTED TO PROVIDE COVERS FOR ALL CONTAINERS.
PREP TABLES, SHELVES, COUNTER TOPS, SIDES OF FOOD EQUIPMENT NOT CLEAN, INSTRUCTED TO CLEAN AND MAINTAIN.
STANDING WATER, MISSING GROUT BETWEEN KITCHEN FLOOR TILES, ROUGH CONCRETE PATCHING, INSTRUCTED MGR TO REPAIR AND CLEAN FLOOR, PROVIDE SMOOTH SURFACE THATS EASY TO CLEAN.
DAMAGED WALLS, WALL BASE TILES ARE IN POOR REPAIR, WALL COVERING NOT MOUNTED PROPERLY, CRACKS, CREVICES, HOLES, OPENINGS NOTED THROUGH OUT FACILITY. INSTRUCTED MGR TO SEAL AND PATCH ALL.

CEILING AND WALL VENT COVERS WITH HEAVY DUST AND GREASE BUILD UP, INSTRUCTED TO CLEAN.
INTERIOR SURFACES OF ICE MAKER NOT CLEAN, SLIME AND DIRT NOTED, INSTRUCTED TO CLEAN AND MAINTAIN.

CRACKS,CREVICES, HOLES STILL REMAINING, ON KITCHEN AND STORAGE ROOMS. INSTRUCTED TO SEAL AND PATCH.
