NO BACK FLOW PREVENTION DEVICE FOR MOP SINK IN REAR AND FOR HOSE IN BASEMENT. MUST PROVIDE. NO VENTILATION IN WAWSHROOM. MUST INSTALL.

DAMAGED WALLS BEHIND THE WASHBOWL SINK IN THE WASHROOM AND WALLS BESIDE THE TOILET BOWL. MUST REPAIR AND MAINTAIN. STAINED CEILING TILES IN THE REPAIR PREP AREA AND IN THE STORE . MUST REPLACE CEILING TILES.
SELF CLOSING DEVICE ON THE DOOR OF THE WASHROOM BROKEN. MUST REPAIR AND MAINTAIN.
MUST SEAL ALL THE HOLES ON THE CEILING AND REPAIR MISSING CEILING ON THE BASEMENT.
FOUND EVIDENCE OF MICE DROPPINGS IN THE BASEMENT ALONG THE WALL BASE AND CORNER OVER 80 TOTAL. INSTRUCTED TO CLEAN AND REMOVE THE DROPPINGS, SANITIZE THE AFFECTED AREA AND INFORM THE PEST CONTROL TECHNICIAN. CITATION ISSUED SERIOUS.
