All employees who handle food shall wash their hands as often as necessary to maintain a high degree of personal cleanliness and should conform to hygienic practices prescribed by the Board of Health.OBSERVED POOR HYGIENIC PRACTICES BY SEVERAL FOOD HANDLERS WITH READY TO EAT PRODUCT (GUM).FOOD HANDLER HAD ON PLASTIC GLOVES OBSERVED HER PULLING AND TOUCHING HER SHIRT AND ALSO HAD A HOLE IN THE GLOVE SHE WAS WEARING AND ANOTHER FOOD HANDLER TOUCHING HAIR RESTRAINT AND HAT ON HIS HEAD AND ANOTHER LIFTING LID ON GARBAGE CAN NOT REMOVING GLOVES OR WASHING HANDS AFTER CONTAMINATION.INSTRUCTED MANAGER TO HAVE THEM TO WASH HANDS AFTER EVERY POSSIBLE CONTAMINATION.
Adequate and convenient toilet facilities shall be provided. They should be properly designed, maintained, and accessible to employees at all times.OBSERVED READY TO EAT FOODS SUCH AS (GUM & BREATH MINTS) BEING HANDLED BY FOOD HANDLERS IN GUM PACKING ROOM,ROLL WRAPPING ROOM AND NO HAND SINK ACCESSIBLE TO ROOM IT IS MORE THAN 40 FEET TO ADDITIONAL HAND SINKS IN ADJACENT ROOMS.INSTRUCTED TO UTILIZE HAND SINKS IN OTHER ROOMS BUT NEED TO INSTALL HAND SINKS IN ABOVE ROOMS.
All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines.OBSERVED EVIDENCE OF APPX. 100 OR MORE LIVE FRUIT FLIES ON WALLS AND CEILINGS IN WOMEN'S TOILET ROOM AND DOORS NOT RODENT PROOF ON TUNNEL DOOR,BACK DOOR,,SOUTH DOCK DOOR.MUST RODENT PROOF DOORS AND RECOMMENDED TO HAVE A PEST CONTROL OPERATOR TO SERVICE PREMISES ON OR AFTER 6/10/10.
All dishwashing machines must be of a type that complies with all requirements of the plumbing section of the Municipal Code of Chicago and Rules and Regulation of the Board of Health.OBSERVED DISH WASHING FACILITIES NOT PROPERLY DESIGNED ONLY A 2-COMPARTMENT SINK ON SITE.OBSERVED SEVERAL BUCKETS BEING WASHED AT 2- COMPARTMENT SINK THAT IS USED TO STORE PRODUCT IN NOT ABLE TO PROPERLY WASH,RINSE AND SANITIZE.INSTRUCTED MANAGER TO INSTALL A 3-COMPARTMENT SINK WITH METAL DRAIN BOARD TO PROPERLY SANITIZE UTENSILS AND BUCKETS.
