All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. OBSERVED 40 OR MORE RODENT (RAT) DROPPINGS ON PREMISES IN BASEMENT ON FLOOR, STORAGE ROOM AND BY REAR EXIT DOOR IN CORNER. FRONT ENTRANCE DOOR NOT RODENT PROOFED AS REQUIRED; MUST RODENT PROOF DOOR. MUST HAVE PEST CONTROL COMPANY SERVICE ESTABLISHMENT PRIOR TO REINSPECTION NEXT WEEK.
Smoking shall be prohibited in all public places except in designated smoking areas. OBSERVED EVIDENCE OF SMOKING ON PREMISES. SEVERAL (25) CIGARETTE BUTTS ON FLOOR OF PREMISES IN STORAGE ROOMS, AREA WHERE THREE COMP SINK LOCATED AND IN BASEMENT ON FLOOR. MUST REMOVE AND REFRAIN FROM SMOKING ON PREMISES PER CITY OF CHICAGO SMOKING ORDINANCE.
Only such poisonous and toxic materials as are required to maintain sanitary conditions may be used in food establishments and they shall not be used in any hazardous manner. OBSERVED TOXIC ITEM OF RODENT BAIT IN A PACKAGE ON FLOOR OF STORAGE ROOM. INSTRUCTED THAT ALL RODENT BAIT MUST BE STORED IN AN ENCLOSED BAIT STATION ONLY; MUST REMOVE.
In all food establishments, toilet facilities shall be kept clean and in good repair and shall include an adequate supply of hot and cold or tempered water, soap, and approved sanitary towels or other approved hand-drying devices. WASHROOMS UNSANITARY. TOILETS,, SINKS AND FLOORS NEED DETAIL CLEANING. MUST PROVIDE HANDSOAP AND HANDTOWELS AT SINK ALSO. CLEAN AND MAINTAIN.

CLEAN INTERIOR AND EXTERIOR OF ALL DISPLAY SHELVES, COOLERS, FREEZERS.
FLOORS IN NEED OF CLEANING TO REMOVE ALL DEBRIS AND BUILD UP THROUGH OUT 1ST AND BASEMENT AREA. MUST CLEAN AND MAINTAIN AT ALL TIMES.
REMOVE ALL UNUSED EQUIPMENT AND UNNECESSARY ARTICLES IN ALL STORAGE AREAS.
REPAIR RIGHT SINK IN BASEMENT WASHROOM. REMOVE WASTE LINE FROM 3-COMP SINK AND CONNECT TO FLOOR DRAIN.
SEAL BRICK WALLS IN BASEMENT STORAGE AREA. REPLACE DAMAGED AND STAINED CEILING TILES IN STORAGE AREAS.
