The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. DETAIL CLEAN FLOORS ALONG WALLS THROUGHOUT PREMISES TO REMOVE DUST AND GRIME.
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. DETAIL CLEAN ALL EQUIPMENT, REFRIGERATION UNITS, COMPRESSORS, PREP TABLES, DOUGH MIXERS ETC THROUGHOUT PREMISES TO REMOVE ACCUMULATED GRIME.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. REMOVE ACCUMULATED ICE UNDER COMPRESSOR IN WALK IN FREEZER AND REPAIR UNIT TO CONTROL SAME.
All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. INCREASE WATER PRESSURE FOR DIPPER WELLS TO PREVENT WATER CONTAMINATION.
The walls and ceilings shall be in good repair and easily cleaned. REPAIR MISALIGNED FRONT DOOR.

FOUND FRONT AND EXIT DOOR NOT RODENT/INSECTS PROOFED WITH 1/4 OF AN INCH GAP AT THE BOTTOM OF THE DOOR. EXIT DOOR WITH 1/4 OF AN INCH GAP ON LEFT AND RIGHT BOTTOM CORNER OF THE DOOR. INSTRUCTED TO RODENT/INSECTS PROOF THE DOORS. ALSO EVIDENCE OF FRUIT FLIES AND HOUSE FLIES INSIDE THE WASHROOM AND SIDE WINDOWS OVER 15 TOTAL. INSTRUCTED TO ELIMINATE FRUIT FLIES AND HOUSE FLIES. CITATION ISSUED SERIOUS.
MISSING DRAIN COVER FOR THE SINK OF THE 3 COMPT SINK AND ONE DRAIN STOPPER FOR THE 3 COMPT SINK IN THE FRONT AREA. MUST PROVIDE.RUBBER GASKET ON THE DOOR OF THE BAGEL OVEN RIPPED OFF. MUST REPAIR AND MAINTAIN. MUST RE CAULK AROUND THE EXPOSED HAND SINK IN THE FRONT AREA, CAULKING WORN OUT.
MUST CLEAN THE FOLLOWING:LID OF THE BULK CONTAINERS OF FLOUR, INTERIOR OF THE MICROWAVE AND MAINTAIN.

OBSERVED STANDING WATER BETWEEN FLOOR TILES,INSTRUCTED TO RE GROUT FLOOR BY AND UNDER THE THREE COMPARTMENT SINKS,ALSO BY THE MOP SINK. RAW WOOD BASE BEHIND THE FRONT COUNTER IN POOR REPAIR AND DIRTY WITH A BUILD-UP OF GRIMES, INSTRUCTED TO PROVIDE A SMOOTH, CLEANABLE AND NON- ABSORBENT MATERIAL. DETAIL CLEAN FLOOR THROUGHOUT THE PREMISES,SPECIALLY UNDER THE COUNTERS,UNDER SHELVES, ALONG BASEBOARD WALLS AND CORNERS(DEBRIS, GRIMES AND BUILD DUST).

Poor hygienic practices. Observed employee behind the front counter cleaning under the finger nails and try to pull a hangnail; took food order from a customer, and handled money and registrar, and without washing hands handled ready to eat products( donuts). OBSERVED WALK-IN FREEZER DOOR NOT ABLE TO CLOSE DUE TO EXCESS ICE BUILD-UP, INSTRUCTED TO REMOVE ICE.
