NEED TO CLEAN THE WIRE RACKS INSIDE THE WALK IN COOLER AND WALK IN FREEZER WITH FOOD STAINED AND FOOD CRUMBS.
MUST REPLACE STAINED DAMAGED CEILING TILES ABOVE THE PEPSI COOLER CLOSE TO THE WINDOWS WEST SIDE. MUST CLEAN IN DETAIL THE VENTS INSIDE BOTH WASHROOM, WITH DUST ACCUMULATED.

Found/Observed front entrance/exit doors not rodent proofed doors had 1/4 inch gaps on bottom of doors,shall be repair.Rear exit door had 1/4inch gaps on bottom and side of door not rodent/insect proofed, shall be repair.Citation issued#H000071326 with a court date of 4/28/2011, at 400 w superior in room #112, at 9:00am.
Exposed raw wood by front counter shelving, counter shall be repair/sealed. All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair.
Non food contact surfaces of cooler shelving, ice machine, and storage shelving not clean need detailed cleaning(crevices). All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily.
Floors under heavy equipment, cooking equipment, and rear storage shelving not clean need detailed cleaning(corners).
Walls, ceilings in rear storage areas need detailed cleaning, ceiling tiles that were damaged/stained, shall be replaced. The walls and ceilings shall be in good repair and easily cleaned.
Missing light shields in doughnut display case shall be provided, light shields in prep areas, storage areas need cleaning. All rooms in which food or drink is prepared, or in which utensils are washed, shall be lighted so that a minimum of 50-foot candles of light is available on all work surfaces. Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities.
