All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. Some storage shelvings above meat prep tables at bucher cooler are rusty- must re-sesl. Peeling caulk at light shield in Bakery W.I.C. must be fixed.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. Floors behind prep tables at baking area, behind display cases must be fre of debris/clutter. Floors of walk-in freezer at bakery area need detailed cleaning also.
The flow of air discharged from kitchen fans shall always be through a duct to a point above the roofline. Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. One handsink at south wall of bakery section is slow-draining, must fix.
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. Following need cleaning: cabinet under 3-comp sink at 'Starbucks' section, delime pot washer machine.

All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments. Over 50 mouse droppings were found around and under shelving in the dog food and snack/cracker aisles. The front sliding doors and the single door outside of the managers office have 1/4' gaps at the bottom and sides, pest proof all outer openings. Remove all evidence of pest activity and clean up all areas of possible harborage.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. THE FLOORS ARE EXTREMELY DIRTY THROUGHOUT THE FACILITY IN THE BATHROOM, UNDER THE SHELVES AND BEHIND THE KICK PLATES. CLEAN AND MAINTAIN THE FLOORS AND BASEBOARDS SO THEY ARE SMOOTH AND SANITARY FROM CORNER TO CORNER.

THE FISH COUNTER ICE MACHINE AND BIN, BREAD DOUGH ROLLER, MILK AND JUICE RACKS, BUTCHER FAN SHEILDS AND BUTCHER WRAPPING MACHINE AND CONVEYOR ARE NOW CLEAN AND THE BREAD DOUGH ROLLER IS BEING REPLACED.
THE FLOORS ARE MUCH CLEANER THROUGHOUT THE FACILITY AND THE DIRTY SHELVING KICK PLATES WERE REMOVED, THE FLOORS UNDER THE SHELVES NEED TO BE STRIPPED DOWN TO THE TILES. CLEAN AND MAINTAIN THE FLOORS AND BASEBOARDS SO THEY ARE SMOOTH AND SANITARY FROM CORNER TO CORNER.
THE BUTCHER SHOP WALLS AND CEILINGS HAVE BEEN CLEANED AND THE RUSTY SPRINKLER HEAD ESCUTCHEONS WERE REPLACED.
