OBSERVED MICE ACTIVITY ON FLOORS, ALONG WALLS AT FOLLOWING AREAS; OVER 15 MICE DROPPINGS NOTED IN MAIN OFFICE STORAGE ROOMS, OVER 60 MICE DROPPINGS FOUND INSIDE TOOL ROOM IN BASEMENT, AND ABOUT 10-15 ADDITIONAL DROPPINGS NOTED IN BASEMENT STORAGE ROOMS. MUST CLEAN-SANITIZE AREAS AND SEAL ANY OPENINGS TO PREVENT PEST ENTRY. ADVICED TO CALL PEST CONTROL FOR SERVICE. CITATION ISSUED 7-38-020 SERIOUS.

THE LIGHT SHIELDS ARE DIRTY AND FULL OF DEBRIS ABOVE THE MAIN KITCHEN READY TO EAT PREP AREA AND THIRD FLOOOR SOUTH SERVING LINE, CLEAN OUT SAID SHIELDS.
THE BOTTOM STEAMER COMPARTMENT HAS SOME FOOD RESIDUE, CLEAN IN DETAIL. THE REFRIGERATOR IN THE STAFF LUNCHROOM IS DAMAGED AND UNSANITARY, REPAIR OR REPLACE SAID UNIT. THE THIRD FLOOR NORTH ICE MACHINE HAS A SMALL AMOUNT OF BUILDUP ABOVE THE DOOR, SAID AREA WAS CLEANED AND SANITIZED AT THIS TIME.
THE CENTER WALK IN COOLER DOOR FRAME IS RUSTY AND UNSANITARY, A WORK ORDER HAS BEEN SUBMITTED FOR SAID REPAIR, REPAIR AREA SO IT IS SMOOTH, SANITARY AND EASY TO CLEAN. THE OPEN SECTION UNDER THE SECOND FLOOR SOUTH SERVING LINE IS DIRTY, CLEAN OUT AND MAINTAIN SAID AREA.
THE THIRD FLOOR SOUTH SERVING LINE HAS A WATER DAMAGED CEILING ABOVE, REPAIR SAID CEILING SO IT IS SMOOTH, SANITARY AND EASY TO CLEAN.
THE THERMOMETER UNIT IN THE THIRD FLOOR SOUTH LOWBOY COOLER IS INACCURATE, REPAIR OR REPLACE SAID UNIT.
THE WASH COMPARTMENT OF THE THREE COMPARTMENT SINK HAS A DIRTY BOOSTER IMPELLER COVER, MAINTAIN IN SANITARY CONDITION OR REMOVE OLD BOOSTER.
