All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. OBSERVED MORE THAN 20-25 RODENT DROPPINGS UNDER CRATES AT JUICE AND SODA POP AREA AND FURNACE ROOM AND UNDER TOILET SINK VANITY. FRONT DOOR IS NOT RODENT PROOFED. ALSO OBSERVED 3-5 DRAIN FLIES. REMOVE RODENT DROPPINGS, CLEAN AND SANITIZE AFFECTED AREAS. RODENT PROOF THE FRONT DOOR. CLEAN ALL FLOOR DRAINS ON PREMISES. RECOMMEND PEST CONTROL SERVICE PREMISES FOR RODENT AND DRAIN FLY ACTIVITY ON OR AFTER 10/26/2011. SERIOUS 7-38-020 CITATION ISSUED.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. NEED A SNEEZE GUARD AT FRONT SERVICE COUNTER. MUST SEAL ALL RAW WOOD AT FRONT SERVICE COUNTER AND WHEREVER NEEDED. MUST REPAIR OR REPLACE ALL RUSTY SHELVING UNITS IN COOLERS, MEAT DISPLAY CASES, AND FOOD SHELVES, AND DOOR HANDLE ON REAR BACK DOOR.

All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily.MUST CLEAN AND REPAINT RUSTY SHELVING UNITS IN COOLERS, MEAT DISPLAY CASES, AND FOOD SHELVES, DEFROST ALL FREEZERS, AND WINDOWS SILLS
