ESTABLISHMENT HAS A SIGNED CONTRACT WITH ANDERSON PEST CONTROL. TECHNICIAN HAS NOT BROUGHT A LOG BOOK WITH THE NECESSARY DOCUMENTS (MSDS SHEETS AND COPIES OF STATE OF ILLINOIS LICENSES) TO ESTABLISHMENT. MUST OBTAIN LOG BOOK. The area outside of the establishment used for the storage of garbage shall be clean at all times and shall not constitute a nuisance. OBSERVED A GREASE DISPOSAL UNIT IN REAR OF PREMISES FULL OF TRASH AND OTHER ITEMS. PREVIOUS OWNER OF ESTABLISHMENT HAD A FIRE AND TRASH WAS PLACED IN THIS UNIT. NEW OWNER HAS A NEW UNIT WITH ANOTHER COMPANY AND HAD BEEN UNSUCCESSFUL IN HAVING OLD UNIT REMOVED. OWNER MADE A CALL AND COMPANY STATED THEY WOULD REMOVE UNIT LATER TODAY. ALSO INSTRUCTED TO REMOVE ITEMS FROM REAR OF ESTABLISHMENT INSIDE ENCLOSED UNIT. ITEMS INCLUDED A SINK, WOOD PIECES, PAPER, TRASH). INSTRUCTED TO REMOVE TALL WEEDS OUTSIDE OF THE ENCLOSED UNIT AS WELL. All food establishments that display, prepare, or store potentially hazardous food shall have adequate refrigerated food storage facilities. ONE REFRIGERATION UNIT NOT MAINTAINING PROPER TEMPERATURE DUE TO SEAL AROUND DOOR BEING IN POOR REPAIR. INSTRUCTED TO REPAIR SO THAT AIR DOESN'T ESCAPE.
