All food establishments that display, prepare, or store potentially hazardous food shall have adequate refrigerated food storage facilities. FOUND LINE COOLER AT 59F WITH FOODS SUCH AS SOUR CREAM AT 53.4F. COOLER TAGGED HELD FOR INSPECTION AND MUST NOT USE UNTIL REPAIRED AND MAINTAINS TEMPERATURE OF 40F OR BELOW, SUPERVISOR SALAZAR CONTACTED AT 312-746-5426 FOR INSPECTION AND TAG REMOVAL.
All cold food shall be stored at a temperature of 40F or less. OBSERVED THE FOLLOWING POTENTIALLY HAZARDOUS FOODS HELD AT IMPROPER TEMPERATURE IN THE ABOVE MENTIONED COOLER. APPROXIMATELY 15LBS CUT CORN AT 55.3F, 10LBS CUT TOMATO AT 53.2F, 1 GL. SALSA AT 50.3F, 5 LBS SOUR CREAM AT 53.4F. ALL FOODS DISCARDED.MANAGEMENT STATES ESTIMATED VALUE TO BE $50.00.
All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. OBSERVED APPROXIMATELY 40 TO 50 RAT DROPPINGS UNDER STORED ITEMS ALONG REAR WALKWAY EXIT TO GARBAGE AREA AND ENTRANCE WAY FOR DELIVERIES. MUST REMOVE ALL DROPPINGS, REMOVE ALL UNNECESSARY ARTICLES FROM REAR WALK WAY AND STORE ALL OTHERS ELEVATED AND ORGANIZED. ALSO FOUND EXIT DOOR FROM OFFICE AT REAR PREP AREA EXITING TO OUTDOOR SIDEWALK NOT RODENT PROOFED WITH OPEN 1/4 INCH GAP ON BOTTOM OF DOOR. INSTRUCTED TO MAKE A TIGHT FITTING SEAL.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. LEAK UNDER THREE COMPARTMENT SINK MUST REPAIR.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. PROVIDE FLOOR DRAIN COVER INSIDE MOP SINK. OBSERVED MISSING/BROKEN BRICKS ON GROUND OF OUTDOOR SIDEWALK PATIO WITH OPEN/EXPOSED DIRT. MUST REPAIR BRICKS AND MAKE CLEANABLE.
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. INTERIOR ICE BIN OF ICE MACHINE MUST CLEAN AND MAINTAIN.
