All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. OVER 50 MOUSE DROPPINGS WERE FOUND IN THE CAFETERIA UNDER THE RAMP TOWARDS THE ENGINEERS SHOP, BEHIND THE DOOR OF THE WEST CAFETERIA HEAT DECK, IN THE THIRD FLOOR TEACHERS LOUNGE PERIMETER AND AROUND THE 2ND FLR CHEMISTRY LAB WERE OLD FOOD ARTICLES WERE LEFT. CLEAN UP ALL EVIDENCE OF MICE AND REPAIR THE REAR GARBAGE DOOR EXIT, THE PEST STRIP AT HE BOTTOM OF THE DOOR WAS TORN OFF. FILL IN THE ACTIVE RODENT HOLES IN THE CAFETERIA BASEBOARDS.
The walls and ceilings shall be in good repair and easily cleaned. REMOVE THE PEELING PAINT FROM THE KITCHEN CEILING.
All parts of the food establishment and all parts of the property used in connection with the operation of the establishment shall be kept neat and clean and should not produce any offensive odors. THE CHEMISTRY LAB IS FULL OF OLD ITEMS WHICH NEED TO BE REORGANIZED.
THE 1-COMPARMENT SINK IN THE SPECIALED ROOM DOES NOT WORK, REPAIR, REPLACE OR REMOVE SAID SINK.

THE CHEMISTRY LAB IS FULL OF OLD ITEMS WHICH NEED TO BE REORGANIZED.
All evidence of mouse activity was removed and the holes were sealed. Alpha Omega pest control also serviced the schoolon Monday.
REMOVE THE PEELING PAINT FROM THE KITCHEN CEILING.
THE 1-COMPARMENT SINK IN THE SPECIAL ED ROOM IS NOW IN GOOD REPAIR.

All utensils shall be thoroughly cleaned and sanitized after each usage. MUST CLEAN-SANITIZE MOP SINKS THRU-OUT PREMISES.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. MUST CLEAN FLOORS THRU-OUT KITCHEN, BOILER ROOM AND MOP CLOSETS.
The walls and ceilings shall be in good repair and easily cleaned. MUST REMOVE PEELING PAINT AND REPAINT, KITCHEN AND MOP CLOSETS WALLS, AND REPAIR WALLS WHERE NEEDED IN MOP CLOSETS.
Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities. MUST REPLACE BROKEN LIGHT SHIELDS THRU-OUT LUNCH ROOM.

The walls and ceilings shall be in good repair and easily cleaned. MUST REMOVE PEELING PAINT FROM WALLS AND CEILINGS OF UTILITY CLOSETS IN HALLWAYS. VARIOUS AREAS OF LUNCHROOM WALLS, KITCHEN DRY STORAGE ROOM AND KITCHEN HAVE AREAS THAT NEED TOUCH UP PAINTING. DRY STORAGE ROOM HAS A SMALL AREA IN NEED OF PLASTER THEN PAINT. REATTACH LOOSE WALL COVING ALONG WALLBASE BEHIND THREE DOOR COOLER IN LUNCHROOM.

THE MOP CLOSETS HAVE DAMAGED AND DIRTY WALLS AND FLOORS, CLEAN AND REPAIR THE CLOSET AREAS.
THE 3RD FLOOR GIRLS WEST WASHROOM HAS ONE SINK OUT OF ORDER, THE 2ND FLOOR GIRLS EAST FAUCET IS DAMAGED, THE LOWER LEVEL EAST GIRLS WASHROOM PLUMBING TO THE SINKS ARE SHUT OFF AND BEING REPAIRED AT THIS TIME. ONE URINAL IN THE LOWER LEVEL BOYS ROOM IS NOT WORKING. REPAIR ALL THE ABOVE PLUMBING. REPLACE THE MISSING STRAINER IN THE REAR CAFETERIA FLOOR DRAIN.
THE FLOOR DRAIN IN THE 3RD FLOOR GIRLS WEST WASHROOM IS MISSING TILES, REPAIR SAID FLOOR SO IT IS SMOOTH AND SANITARY.
THE ADA DOOR SWEEP ON THE SOUTH EAST CORNER OF THE BUILDING IS BECOMING WORN, REPLACE THE SWEEP.
THE WALK IN COOLER HAS DIRTY CEILINGS, RACKS AND FAN GUARD; WAH, RINSE AND SANITIZE THE ABOVE IN DETAIL.
THE WALLS ARE DAMAGED BETWEEN THE WALK IN COOLER AND FREEZER DOORS, THE PREP TABLE WITH THE GREEN PAINT ON IT IS PEELING, STRIP OFF THE PEELING PAINT.
