OBSERVED BLACK AND PINK MOLD GROWTH ON INTERIOR SURFACES OF THE ICE MACHINE. UNIT TAGGED BY CDPH. INSTRUCTED MANAGEMENT TO TURN OFF MACHINE, REMOVE ICE AND HAVE INTERIOR OF MACHINE CLEANED AND SANITIZED. FAX REINSPECTION LETTER TO (312) 746-4240 AFTER UNIT HAS BEEN CLEANED. SERIOUS VIOLATION 7-38-005A.

MANAGEMENT VOLUNTARILY DISCARDED 757# OF FOOD WORTH $1566. CRITICAL VIOLATION 7-38-005A.
