All parts of the food establishment and all parts of the property used in connection with the operation of the establishment shall be kept neat and clean and should not produce any offensive odors. ORGANIZE AND MAINTAIN ALL STORAGE AREAS.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. MUST CLEAN AND MAINTAIN CUTTING BOARDS IN PRODUCE PREP AREA.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. MUST CLEAN FLOORS THROUGH OUT STORAGE ROOM BEHIND TRASH COMPACTOR, UNDER STORAGE SHELVES AND BEHIND OTHER EQUIPMENT. ALSO CLEAN IN ALL WALK-IN COOLERS UNDER SHELVES. ADVISED TO MAINTAIN ALL FLOOR DRAINS AS WELL. SOME SECTIONS OF FLOOR IN POOR REPAIR IN MEAT PROCESSING AREA WHERE GRINDER LOCATED, MUST REPAIR.
The walls and ceilings shall be in good repair and easily cleaned. Use dustless cleaning methods. MUST CLEAN FAN GUARD COVERS IN MEAT COOLER TO REMOVE DUST OBSERVED. INSTRUCTED TO CLEAN AND/OR REPLACE PLASTIC DOOR FLAPS WHERE NEEDED AT COOLER DOORS AND REAR RECEIVING DOOR. SOME AREAS OF WALLS IN WALK-IN COOLERS HAVE CHIPPED PAINT, WALLS NEED CLEANING AND REPAINTING.
WASHROOM DOORS MUST HAVE SELF-CLOSING DEVICES INSTALLED.

The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. THE FOLLOWING NOT CLEAN FLOOR IN THE BASEMENT,BACK STORE ROOM, AROUND THE TRASH COMPACTOR AND UNDER THE STORAGE SHELVES IN THE WALK IN COOLERS. INSTRUCTED TO CLEAN SAME.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. MANAGER DISPOSED OF THE MEAT AND WERE DENATURED WITH BLEACH.120 LBS COST $166.80 CRITICAL CITATION GIVEN 7-38-005(A)

THE CHICKEN COOLER RACK SUPPORTS HOLD A QUANTITY OF LIQUID AND CANNOT SELF DRAIN, REPAIR OR REPLACE THESE SUPPORTS SO THEY ARE SANITARY AND DRAIN OFF LIQUIDS.
THE KITCHEN KNIFE RACK CONTAINS A DIRTY KNIFE AND SHARPENING STEEL; WASH, RINSE AND SANITIZE THE KNIVES AND TOOLS PRIOR TO STORAGE.
THE CHICKEN COOLER FLOOR DRAIN AREA IS BUCKLING AND HOLDS WATER, REPAIR THE FLOOR SO IT IS SMOOTH AND SANITARY.
THE WALL BEHIND THE BALER AND THE SOUTH BUTCHER SERVICE AREA CEILING ARE DIRTY, CLEAN AND RESEAL SAID AREAS.
THE BUTCHER PREP AREA HAS TWO MISSING LIGHT SHIELDS, REPAIR SO THE LIGHTING IS SHIELDED.
THE ICE MACHINE DOOR INTEROIR IS DIRTY; WASH, RINSE AND SANITIZE THE ICE MACHINE INTERIOR.
