Found/observed front entrance/exit door not rodent proofed, rear exit doors in rear kitchen areas not rodent proofed, all doors must be rodent proofed in order to pass license inspection, no citation issued due to license inspection for task force on 4/19/2011.
Dishmachine not working at time of license inspection for task force on 4/19/2011, must be in good repair, and operating in order to pass inspection, no citation issued due to license inspection for task force on 4/19/2011. Provide chlorine test strips. Remove sealant from drains in 3 compartment and 1 compartment sinks.
Walk in cooler shelving, and storage shelving in storage areas that had peeling paint, rust shall be replaced/repair. Exposed raw wood underneath front counter/bar area shall be repair/sealed.
Non food contact surfaces of walk in cooler shelving, coolers, storage shelving, deep fryer not clean need detailed cleaning.
Floors under heavy equipment, cooking equipment, storage areas, bathrooms, dining areas not clean need detailed cleaning. Damaged floor tiles in rear storage areas shall be repair/replaced.
Damaged ceiling tiles in rear kitchen, storage areas shall be replaced, damaged ceiling in rear kitchen areas shall be repair/sealed.
Missing light shields in front prep/bar area shall be provided, missing light shields, and end caps of light shields in rear kitchen areas shall be provided.Light shields need cleaning in prep areas, storage areas.
Employee's bathroom self closing mechanism in state of disrepair, shall be repair/replaced.
Exhaust canopy missing filters, must provide filters to exhaust canopy.
Post required 'no smoking' signs at entrances and other areas where smoking is prohibited.

Floors under heavy equipment, cooking equipment, behind hot water heater not clean need detailed cleaning(corners), floors in walk in walk in cooler not clean need detailed cleaning(corners), missing/damaged floor tiles in rear kitchen areas, prep area shall be reapired/sealed
Exhaust vents(ventilation)in walk in cooler, kitchen prep area, and hallway outside bathrooms not clean need detailed cleaning.

Stock items must be stored six inches off of floor to prevent pest harborage.
