All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. OBSERVED FRUIT/DRAIN FLIES IN THE FOLLOWING AREAS: BEHIND FRONT DINING BAR, IN FRONT AND REAR DINING ROOM, IN MENS WASHROOM AND HALLWAY TO WASHROOM ADJACENT TO FRONT DINING ROOM AND IN REAR DISH AREA AT UTILITY MOP SINK. INSTRUCTED TO HAVE PEST CONTROL CO. FURTHER TREAT FOR FLIES, REMOVE ALL MOIST ORGANIC MATERIAL FROM FLOORS OF ALL AFFECTED AREAS, SEAL ALL OPEN WET CREVICES TO MINIMIZE OR ELIMINATE ACTIVITY. APPROXIMATLEY OVER 30 FLIES.
One copy of the Food Inspection Report Summary must be displayed and visible to all customers. SUMMARY REPORT FROM 6-19-09 #161359 STATING THAT CRITICAL VIOLATIONS HAD BEEN CORRECTED DURING THAT INSPECTION WAS NOT POSTED IN PLAIN VIEW OF ITS CUSTOMERS. REPORT WAS FOUND STORED INSIDE PREVIOUS REPORTS.
A separate and distinct offense shall be deemed to have been committed for each Minor violation that is not corrected upon re-inspection by the health authority. CLEAN WALL AT UTILITY MOP SINK. REPAIR WALL, SEAL ALL OPENINGS BEHIND PIZZA OVEN AND AT PREP AREA.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. BAR ICE BIN MUST PROPERLY DRAIN TO FLOOR DRAIN.
Containers and utensils shall be inverted, covered, or otherwise protected from contamination until used. All single-service drinking straws and containers shall be discarded immediately after use. KNIFE FOUND IMPROPERLY STORED BETWEEN SINK AND WALL AT FRONT BAR. MUST PROPERLY STORE. ALL TO GO CONTAINERS STORED IN BASEMENT AND PREP MUST BE INVERTED.
The walls and ceilings shall be in good repair and easily cleaned. DAMAGED WALL AT ENTRANCE TO KITCHEN AND STAIRWELL TO BASEMENT.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. MUST NOT USE CARDBOARD AS MATTING FOR FLOORS.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. MUST NOT USE TINFOIL OR CARDBOARD AS LINER FOR SHELVING.
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. DISH RACKS USED FOR DISH WASHING AT DISH MACHINE STAINED BLACK WITH ACCUMULATE DIRT, MUST REPLACE.
Between uses and during storage ice dispensing utensils and ice receptacles shall be stored in a way that protects them from contamination. COOKS LINE WIPING CLOTHS MUST BE STORED IN A CLEAN CONTAINER WITH A SANITIZING SOLUTION.

REPLACE THE TORN DOOR GASKET INSIDE OF THE 1 DOOR PREP COOLER.
REPAIR THE SINK STOPPER LEVERS FOR THE MIDDLE AND RIGHT COMPARTMENTS OF THE 3 COMPARTMENT SINK IN THE PREP AREA SO THAT THEY BOTH CAN HOLD WATER IN THE SINK BASIN.
