Aug 11, 2011

Storage shelving that had peeling paint/rust shall be repair/replaced, Cooler shelving that had peeling paint/rust shall be repair/sealed.Exposed insulation in walk in cooler by front doors shall be repair/sealed.
Non food contact surfaces of cooler shelving/racks not clean need detailed cleaning(crevices), Storage shelving racks throughout premises not clean need detailed cleaning.
Floors in walk in cooler rear storage areas, and throughout premises not clea need detailed cleaning(corners). Floors that had broken floor ,exposed cement shall be repair/sealed.
Notice evidence of rodents droppings under display shelving areas, must update pest control service contract under new ownership and provide current IDPH pest control license, all exterior doors must be adjusted as to be rodent/insect proofed per City of Chicago Health Codes. Advised to completely seal gap at bottom of door to next storefront. No citation issued due to license inspection.
All walls, ceiling in rear storage rooms, areas that had peeling paint, peeling plaster, exposed wallboard shall be repair/sealed, exposed insulation in rear storage room shall be repair/sealed.Walls, ceilings that had exposed plaster, peeling paint throughout premises, shall be repair/sealed.Must provided cove moulding in mop room , and along wallbase in rear storage areas throughout premises.
Missing light shields in walk in coolers , shall be provided, light shields need cleaning(detailed cleaning).
Missing vent guard in walk in cooler shall be provided, exhaust vents(ventilation)in walk in cooler, premises not clean need detailed cleaning.
Must provide thermometers to coolers in order to maintain proper temperatures of 40f or less.
Excess debris and clutter in rear storage rooms shall be removed, all unused equipment, equipment that is not in working order shall be removed, stock items must be stored six inches off of floor to prevent pest harborage.

CLEAN, REMOVE OR ORGANIZE CLUTTER AT THE REAR STORAGE AREA TO PREVENT HIDING PLACES FOR RODENTS.
NOTED LEAKAGE AT REAR CEILING AND MULTIPLE HOLES AT SAME. MUST REPAIR AND MAINTAIN.... ALSO SEAL HOLE ON WALL OF SAME.
INSTRUCTED TO DETAIL CLEAN FLOOR AT THE REAR STORAGE ALONG WALL BASE AND MAINTAIN DRY FROM LEAKING WATER FROM THE CEILING.
INSTRUCTED TO CLEAN FANS INSIDE THE WALK IN COOLER
