OBSERVED INTERIOR OF ICE MACHINE WATER RESERVOIR WITH BLACK SLIME SUBSTANCE DRIPPING INTO WATER WHERE ICE IS MADE,ALSO SLIME SUBSTANCE WAS OBSERVED INSIDE THE ICE NOOZLE DISPENSER AND TRAYS,INSTRUCTED TO CLEAN AND SANITIZE UNIT,TURNED OFF THE UNIT,TAGGED ICE MACHINE'HELD FOR INSPECTION' DO NOT USE- FAX A LETTER WHEN UNIT IS CLEANED 312-746-4240,CRITICAL VIOLATION 7-38-005(A),H000077634-11
FOUND BOTH WASHROOMS FIRST FLOOR BY THE FRONT DESK USED BY THE STAFF AND RESIDENTS WITHOUT PAPER TOWELS,OBSERVED A MAN COMING OUT FROM MEN'S WASHROOM TRY TO DRY HANDS ON SLEEVES,INSTRUCTED TO PROVIDE PAPER TOWELS OR HAND DRYING DEVICE IN BOTH WASHROOMS,PAPER TOWEL PROVIDED UPON MY REQUEST-CRITICAL VIOLATIONS 7-38-030. FOUND STAFF WASHROOM NOT MAINTAINED,WE OBSERVED(MANAGER AND I) ENCRUSTED FECES ON TOILET SEAT,SEAT AND TOILET HAS BEEN CLEANED UPON MY REQUEST,CRITICAL VIOLATION 7-38-030.
