Non food contact surfaces of cooler shelving/racks that had peeling paint, rust shall be repair/replaced.Exposed raw wood in bar counter areas, shall be repair/sealed.
Non food contact surfaces of ice machine, cooler shelving, cooking equipment, and storage shelving not clean need detailed cleaning(crevices).Bsmt. storage racks need cleaning(detailed).
Floors under heavy equipment, cooking equipment, storage shelving in storage room not clean need detailed cleaning(corners).Floors in bsmt. storage areas under storage shelving/racks not clean need detailed cleaning(corners).
Walls that had exposed wallboard, plaster in bathrooms shall be repair, sealed. walls that had holes gaps in rear kitchen, storage areas shall be repair.
Light shields in kitchen prep area need cleaning, missing light shields in bsmt. storage areas shall be provided.
Exhaust canopy not clean needs detailed cleaning, exhaust vents(ventilation)in kitchen prep areas, dining areas, and bathrooms need detailed cleaning.Exhaust fan in bsmt, 1st floor in state of disrepair, shall be repair/replaced.

Dish machine not sanitizing properly (0 PPM chlorine). Dish machine tagged 'held for inspection' at this time of inspection. Mgmt. was advised to contact technician to repair. No citation issued due to dish machine not in use, set-up upon request.
Found ice machine(interior of ice machine)food contact surfaces with pink, black slime above ice machine chute, condensation dripping onto ice. Citation issued H000072750 , mgmt was instructed not to use ice machine, and to contact ice machine company to clean and sanitize all of the interior of ice machine.
Found/observed front entrance, rear entrance exit doors not rodent proofed, doors had gaps on bottom , and side of doors ranging from 1/2 inch, to 1/4inch gaps/openings, shall repaired/sealed all gaps on all doors, also found/observed rodent droppings(20+)in rear storage area by mixer, Citation issued H000072750. Must remove all droppings, wash and sanitize surfaces. Recommended additional service by pest control company.
Must provide splash guard between mop sink and utensil storage shelving. Walk in cooler racks that had rust, shall be repaired, all exposed raw wood in bar areas shall be repaired/sealed.
Found some food items not properly dated/labeled in coolers, reach in freezer, must date and label food items in coolers, reach in freezer.
Non food contact surfaces cooler shelving, knife rack, not clean need detailed cleaining, ice machine needs detailed cleaning.
Floors under heavy equipment, cooking equipment not clean need detailed cleaning, floors in rear storage areas by mixer not clean needs detailed cleaning(corners), floors in both bsmts, not clean need detailed cleaning(corners, excess dirt/debris removed.
Walls that had peeling paint in bathrooms, shall be repaired/sealed, damaged/stained, ceiling tiles shall be replaced/repaired, hole in bathroom ceiling shall be repaired/sealed, walls that had gaps/openings shall be repaired/sealed.
Missing light shield in walk in cooler shall be provided, missing light shields in rear kitchen prep area shall be provided.
Missing exhaust fan in bathroom, shall be provided, exhaust fans in state of disrepair, or broken in bathrooms shall be replaced/provided. exhaust canopy not clean needs detailed cleaning. Leaking overhead pipe in basement. Must repair leak, clean/dry floor. A hose may not be attached to any faucet unless a backflow prevention device is installed. Must provide at basement hose bibb. Must provide exposed hand sink to bar area on east side(banquet room).
Must provide thermometers to all coolers, to properly maintain 40f or less at all times.
Excess debris and clutter in basement storage areas shall be removed, stock items must be stored six inches off of floor to prevent pest harborage.
