Exposed handsink in ice cream area not working. Instructed to repair sink with hot and cold running water or remove. Premieses does have additional hand sink that is near the ice cream bar.
Outside garbage waste grease area not maintained or cleaned. Observed waste grease and encrusted grease on exterior of grease box. Also, observed waste grease on ground around grease box. Instructed manager that the grease box exterior and the ground must be kept cleaned, dry and maintained to prevent any pest activity. Serious citation issued 7-30-020.
Potentially hazardous foods doesn't meet temperature requirements during storage preparation, display and service. Found the following potentially hazardous foods stored in the cooler stated above: Tarter sauce 1 gal at 45.3F, Caesar dressing 1 gal at 46.2F, Feta cheese 5 gal at 50.0F, Mozzarella cheese 2 qrts at 44.4F. Instructed that all potentially hazardous foods must maintain 40F or below during storage. Manager discarded all products value $80.00, 8 gallons. Critical citation issued 7-38-050A.
Facilites not maintaining proper temperature. Found condiment prep in rear prep area at improper temperature. Internal temperature at 47.1F, while storing potentially hazardous foods. Instructed manager that the internal temperature must maintain 40F or below at all times potentially hazardous foods are being stored. Critical citation issued 7-38-005A. 'Held for inspection' tag placed on cooler.
Previous minor violations not corrected: 6-17-2009, report# 159665: #30-Foods stored in coolers not labeled or dated. Instructed to label and date all foods not in original container. #33-Storage shelves, all cooking equipment, sinks, deep fryer, cabinets and prep tables/counter not cleaned. Instructed to detail clean and sanitize daily. #34-Floor thru-out not cleaned and in poor condition. Instructed to detail clean and replace missing and broken floor tile. Serious citation issued 7-42-090

Foods inside coolers not labeled. Instructed to label all foods with name thats not in original containers.
Evidence of rodent activity found on premises. Found mice dropping in side storage room approximately 15 dropping and in rear prep area along utility sink right side approximately 5 and in corner of walk-in freezer appromately 10. Instructed to remove all evidence of mice activity, detail clean all areas. Must also repair lower right corner of rear door, to prevent rodent entry. Serious citation issued 7-38-020.
Floor through-out premises not cleaned. Instructed to detail clean and remove food debris daily. Floor tile damaged in rear. Instructed to replace broken floor tile.

Found potentially hazrdous foods no meeting temperatures of 40f or less. Found Roast beef at 45.3f, Chicken Breast at 43.7f, Pasta at 44.3f, in reach in cooler, mgmt discarded product at once, aprox. value $120.00, aprox. weight.25 lbs.
Found/observed front entrance/exit door not rodent proofed, door had 1/4 inch gap, shall be repair.Citation issued #H000071331, with a court date of 5/26/2011, at 400 w superior in room #112 at 9:00am.
Cooler shelving, racks that had peeling paint, rust shall be repair/replaced.
Non food contact surfaces of walk in cooler shelving, prep tables (lower shelving)not clean need detailed cleaning(corners), rear storage shelving/racks not clean need detailed cleaning(crevices).Front counter storage shelving needs detailed cleaning.
Floors under heavy equipment,cooking equipment, 3 compartment sink, storage shelving, and by front counter not clean need detailed cleaning(corners).
Walls/ceilings in rear kitchen prep areas not clean need detailed cleaning.Front prep area wall need detailed cleaning.
Missing, broken light shield were missing in rear kitchen prep area,shall be replaced. light shields in prep areas not clean need detailed cleaning.
Exhaust canopy had excess grease build, needs detailed cleaning. Exhaust vents(ventilation) in kitche prep areas not clean need detailed cleaning.
