In all food establishments, toilet facilities shall be kept clean and in good repair and shall include an adequate supply of hot and cold or tempered water, soap, and approved sanitary towels or other approved hand-drying devices. FOUND NO SOAP IN MEN'S TOILET AND NO PAPER TOWEL IN HANDSINK,SERIOUS VIOLATION,CITATION ISSUED,MANAGER PROVIDED SOAP AND PAPER TOWEL;INSTRUCTED TO PROVIDE SOAP AND PAPER TOWEL AT ALL TIMES
Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities. BROKEN LIGHT SHIELD IN COOKING AREA MUST BE REPLACED
The flow of air discharged from kitchen fans shall always be through a duct to a point above the roofline. VENTS IN LADIES TOILET MUST BE FIXED
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. MUST READJUST DOOR SWEEP IN ENTRANCE DOOR TO PROPERLY SEAL DOOR
The walls and ceilings shall be in good repair and easily cleaned. CEILING TILES IN DINING AREA AND PREP AREA WITH DUST ACCUMULATION,MUST CLEAN AND MAINTAIN
All food establishments that display, prepare, or store potentially hazardous foods shall have calibrated metal stem thermometers, provided and conspicuous, for refrigerated and hot food units. MUST PROVIDE THERMOMETER IN COOLER FRONT AREA

Exhaust vents(ventilation) in walk in cooler, coolers need cleaning, must provide exhaust fan to bsmt bathroom.
Found/observed front entrance door not rodent proofed, door had 1/2 inch gap on bottom of door, also found/observed rear kitchen exit door had 1/4 inch on bottom of door.Citation#H000072749 issued.
Found food items not properly dated/labeled in coolers, must date and label food items in coolers.
Cooler shelving that had peeling paint/rust, shall be repaired/replaced.
Non food contact surfaces of walk in cooler shelving, cooler shelving not clean need detailed cleaning(crevices), non food contact surfaces of ice machine not clean need cleaning.
Floors in bsmt storage areas not clean need detailed cleaning(corners), damaged/broken floor tiles in rear storage area(1st floor)shall be replaced.
Walls that had peeling paint, exposed plaster, exposed wood, in rear storage areas(1st floor, bsmt.)shall be repaired/sealed, damaged ceiling tiles in rear prep area shall be replaced.
Missing light shield in walk in cooler, diplay cooler, shall be provided/replaced.
