EVIDENCE OF MICE ACTIVITY NOTED ON PREMISES DURING INSPECTION OBSERVED ABOUT 20-45 MICE DROPPINGS ON FLOORS AND IN SIDE CABINETS UNDER COUNTER IN THE BAR AND IN STORAGE ROOMS AT CORNER REAR. NO PEST CONTROL SERVICES,NOR LOG FOR REVIEW FRONT DOOR NOT PROPERLY RODENT PROOFED ABOUT 1 1/2'- 2' GAP ON SAID DOOR,OPENING ON FLOORS IN BAR,OUT SIDE CORNER ENTRANCE DOOR AND IN FURNACE ROOM. CITATION ISSUED 7-38-020 SERIOUS. INSTRUCTED OWNER TO CLEAN MICE DROPPINGS AND SANITIZE AFFECTED AREAS,CONTRACT A LICENSED PEST CONTROL COMPANY TO SERVICE FACILITY AND PROVIDE LOG,PROPERLY RODENT PROOF DOOR,AND SEAL ALL OPENINGS ON FLOORS. WILL BE BACK FOR RE-INSPECTION ON 06-23-10.
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. MOP SINK,INTERIOR OF REACH IN COOLER IN THE MOP SINK ROOM, MICROWAVE OVEN, AND COUNTERS IN SAME LOCATION WITH DEBRIS NEEDS CLEANING.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. FLOORS IN BASEMENT WITH STADING WATER DUE TO LEAKING PIPE MUST BE CLEANED AND MAINTAIN DRY AT ALL TIMES,CLEAN FLOORS IN WALK IN COOLER . REPLACE MISSING FLOOR TILES IN AND STORAGE ROOMS.
The walls and ceilings shall be in good repair and easily cleaned. REPAIR WATER DAMAGED CEILING TILES IN STORAGE ROOMS, IN TOILETS AND BASEMENT.
None of the operations connected with food establishments shall be conducted in any room used as living or sleeping quarters. Toilet rooms shall be completely enclosed and shall be vented to the outside air or mechanically ventilated. MUST REPLACE MISSING SELF CLOSING DEVICE ON MALE TOILET DOOR
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. MUST REPAIR LEAKING PIPE IN BASEMENT,REPAIR COLD WATER KNOB ON MOP SINK EXPOSE HAND SINK IN BAR,AND LEAKING FAUCET ON 3COMP. SINK IN EAST STDE CORNER BAR.

MUST REMOVE UNNECESSARY AND UNUSABLE ARTICLES IN CABINETS,WALK IN COOLER,MOP SINK AND FURNACE ROOM,ETC. FROM FACILITY,INCLUDING GRILL, AND POTS.
