Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. Leaky faucet at 3-comp sink must be fixed.
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. Interior bottom of fryers & interior panel of ice machine need cleaning.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. Broken floor tiles by the door entry threshold & below the handsink must be replaced/fixed.

All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. THE WALK IN COOLER DRAIN PAN LEAKS INTO A BUCKET, REPAIR THE DRAIN PAN SO THE CONDENSER WATER FLOWS DOWN A SANITARY DRAIN TUBE. THE NORTH SANDWICH PREP COOLER HAS A DIRTY INTERIOR; WASH, RINSE AND SANITIZE ALL OF THE FOOD DISPENSING UTENSILS AND EQUIPMENT UNTIL IT IS CLEAN, SANITARY AND MAINTAINED ON A DAILY BASIS.
Between uses and during storage ice dispensing utensils and ice receptacles shall be stored in a way that protects them from contamination. THE ICE SCOOP IS STORED ON TOP OF THE ICE MACHINE, STORE IT IN A SANITARY HOLDER OR CONTAINER.

ALL FOOD HANDLER'S MUST WEAR HAIR RESTRAINTS AT ALL TIMES.
MUST RECAULK WHERE WALL MEETS THE TOP OF THE 3COPM SINK SO AS TO BE SMOOTH AND EASILY CLEANABLE. MUST ALSO REPAIR CRACKED CERAMIC WALLBASE COUVING AT THE CORNER OF THE WALK-IN FREEZER.
MUST REPAIR LOOSE GASKET AT THE DOOR OF THE WALK-IN COOLER.
MUST REPAIR CRACKED LIGHT SHIELD ABOVE SANDWICH PREP AREA.
