Violation descriptions and comments

A certified food service manager must be present in all establishments at which potentially hazardous food is prepared or served. NO CERTIFIED MANAGER PRESENT DURING INSPECTION WHILE POTENTIALLY HAZARDOUS FOODS ARE BEING PREPARED AND SERVED.
Containers and utensils shall be inverted, covered, or otherwise protected from contamination until used. All single-service drinking straws and containers shall be discarded immediately after use. IMPROPER SCOOP BEING USED FOR FILLING SMALL SALSA CONTAINERS. MUST USE LADLE OR SCOOP WITH HANDLE. NO BARE HANDS ON STYROFOAM THEN STORED INSIDE SALSA.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. MUST REMOVE THREADED NOZZLE FROM UTILITY MOP SINK OR INSTALL BACK FLOW PREVENTION DEVICE. LEAK AT LEFT SIDE NOZZLE AT THREE COMPARTMENT SINK.
The walls and ceilings shall be in good repair and easily cleaned. WATER DAMAGED CEILING TILES IN REAR, MUST REPLACE. MISSING AND MOLDY WALL CAULKING AT THREE COMPARTMENT SINK, MUST REPLACE.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. CRACKED, DAMAGED AND MISSING FLOOR TILES IN WALK IN COOLER AND OF THRESHOLD OF COOLER, MUST REPLACE/REPAIR.
Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities. CRACKED LIGHT SHIELD IN FOOD PREP, MUST REPLACE.
All food not stored in the original container shall be stored in properly labeled containers. ALL PREPARED FOOD IN WALK-IN COOLER MUST BE LABELED AND DATED.
Only employees must dispense ice for consumer use with scoops, tongs, or other ice dispensing utensils. Customers must dispense ice through appropriate automatic self-service ice dispensing equipment. ICE SCOOP MUST BE PROPERLY STORED IN A CLEAN CONTAINER, NOT ON TOP OF MACHINE.

All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. OBSERVED A STYROFOAM CUP BEING USED AS A SCOOP INSIDE OF A LARGE CONTAINER OF SALSA STORED INSIDE OF THE WALK-IN COOLER. MUST USE A PROPER UTENSIL SUCH AS A SPOON TO DIP IN THE SALSA.
Between uses and during storage ice dispensing utensils and ice receptacles shall be stored in a way that protects them from contamination. OBSERVED THE ICE SCOOP STORED INSIDE OF THE COOLER USED AS AN ICE CHEST. MUST REMOVE AND STORE IN A CLEAN CONTAINER OUTSIDE OF THE COOLER.
