All employees shall be required to use effective hair restraints to confine hair. HAIR RESTRAINTS ARE NEEDED FOR BAKERY EMPLOYEES.
All parts of the food establishment and all parts of the property used in connection with the operation of the establishment shall be kept neat and clean and should not produce any offensive odors. REMOVE ANY UNUSED MATERIALS AND EQUIPMENT FROM WAREHOUSE AREA BEHIND THE BAKERY.
All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. EVIDENCE OF RODENT ACTIVITY OBSERVED ON PREMISES. OBSERVED APPROX. 100 MOUSE AND RAT DROPPINGS ON THE FLOOR OF THE WAREHOUSE AREA BEHIND THE BAKERY. ALSO OBSERVED 5 MOUSE DROPPINGS ON THE FLOOR IN THE BAKERY AND 2 DEAD MICE IN A TINCAT TRAP IN THE BAKERY. MANAGEMENT INSTRUCTED TO CLEAN ALL DROPPINGS AND SANITIZE AFFECTED AREAS. ADDITIONAL PEST CONTROL SERVICE IS NEEDED TO MINIMIZE PEST ACTIVITY. SERIOUS VIOLATION 7-38-020.

Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities. LIGHT SHIELDS WITH END CAPS ARE NEEDED IN THE BAKERY PREP AREAS. LIGHT SHIELDS NEEDED FOR THE WALK-IN COOLER.
