Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. 2-compartment sink faucet leaking at base. Must repair and maintain.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. Observed prep coolers rubber door gaskets ripped. Instructed manager to repair/replace all damaged coolers gaskets.
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. Must clean interior of all unclean coffee cabinets at front and all unclean coolers condensers.

The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. OBSERVED LIQUID BUILD UP ON FLOOR AROUND 2-COMPARTMENT SINK GREASE TRAP. INSTRUCTED MANAGER TO CLEAN/KEEP FLOOR DRY AROUND GREASE TRAP.

MUST DETAIL CLEAN AND MAINTAIN FLOORS IN CORNERS AND UNDER/AROUND ALL HEAVY EQUIPMENT THROUGH OUT.
MUST FIX THE LEAK AT THE BASE OF THE RIGHT FAUCET ON THE 3-COMP. SINK IN THE REAR PREP AREA.
MUST REROUTE THE EXPRESSO DRAIN LINE FROM THE 1-COMP. SINK TO A FLOOR DRAIN IN THE FRONT PREP AREA.
OBSERVED A FOOD HANDLER USING A GARBAGE CAN AS A PREP TABLE, HE PUT A TRAY ON TOP OF THE GARBAGE CAN AND THEN STARTING PUTTING CHICKEN ON TO THE TRAY. MANAGER DISCARDED FOOD ITEM AND TOOK TRAY TO REAR FOR WASHING. INSTRUCTED TO USE PREP TABLES FOR PREPARING FOOD AND WASH HANDS WHEN TOUCHING GARBAGE CANS, ETC. CRITICAL VIOLATION 7-38-005(A)

MUST PROVIDE A WORKING THERMOMETER AT THE COLD HOLDING UNIT ACROSS FROM THE CASH REGISTER.
MUST REGROUT FLOOR AT REAR PREP AREA IN FRONT AND UNDER THE 3COMP SINK AND UNDER AND IN FRONT OF EXPOSED HAND SINK.
MUST REPAIR GASKETS AT THE 2DR COOLERS IN THE REAR FOOD PREP AREA.
MUST FIX THE LEAK AT THE BASE OF THE FAUCET ON THE 2-COMP. SINK IN THE REAR PREP AREA.
