KDA_2_30115 - --The food service employee was washing her hands at the dish sink. The soap and drying towels were located at the dish sink and not the hand sink in the kitchen. COS. Educated staff. Moved soap and towels.
KDA_3_30511A1 - --There was one case of bananas on the floor in the kitchen.
KDA_3_30611 - --The self-serve buffet area did not have protection covering the exposed food items. COS. Educated staff. Items were covered with plastic film.
KDA_3_30613B - --Located at the self-serve bread station, there was not any tongs or other serving utensil and the consumers were reaching inside the bags of bread with their bare hands. COS. Tongs are provided. Educated staff about possibly individually wrapping each bread item if tongs do not seem to work.
KDA_3_50118A2 - --The bulk milk machine had 2 open bladders of milk with no open date. PIC said they were opened on Monday, October 12. COS. Milk was dated.
KDA_4_20211A2 - --There were 4 out of 15 chipped cereal bowls sitting on the shelving with clean dishes. COS. Bowls were placed in trash can.
KDA_4_30212A - --There was not a food thermometer in the kitchen or FE. COS. Provided a thermometer and PIC has them ordered from supply company.
KDA_4_50213B - --The bulk milk machine tubing was not cut on the diagonal and was cut more than 1 inch. COS. Tubing was cut correctly. NOTE: The bulk milk machine is located in a potentially hazardous area. The machine sits directly above the ice machine and dried milk has dribbled on the lid of the ice machine and has potential to dribble inside and onto the ice.
KDA_4_60111C - --The pot holders all have an accumulation of greasy residue.
KDA_4_90311A - --There was one case of forks and one case of spoons sitting on the floor in the kitchen.
KDA_5_50117 - --There are not any covers on the trash cans in the unisex toilet rooms designated for food service.
KDA_6_20214 - --The unisex toilet room doors are not self closing.
