Corrected on site (COS)- moved hashbrowns. KDA_3_30212 - --Located on the dry storage shelf in the kitchen, there were two unlabeled containers of white powder which the PIC identified as flour and sugar. COS- labeled. KDA_3_50118A2 - --Located in the Frigidaire RIC in the kitchen, there was an opened gallon of chocolate milk that had no date marked on it. Person in charge (PIC) indicated that it had been opened 9/18. COS- dated. KDA_3_60311B - --The menu did not have asterisks next to foods that are available to be cooked to order (eggs). There was a disclosure statement at the bottom of the menu. COS- made consumer advisory sign and posted it on wall that eggs are available to be cooked to order. KDA_4_10111A - --Located in the storage area, there was a non food grade sterilite container with sugar inside. COS- put sugar in plastic food bags and put back into container. KDA_4_50111B - --The lid to the kitchen chest freezer is broken. KDA_4_60111A - --Located in a cup with utensils on the kitchen island counter, there were two metal spatulas, two ice cream scoops, and one pair of kitchen shears stored as clean that were soiled with dried food debris. COS- moved to wash area. Located on a shelf under the kitchen island counter, there were two metal food containers stored as clean that were soiled with dried food debris. COS- moved to wash area. Located in a drawer under the kitchen island counter, there were two metal measuring cups stored as clean that were soiled with dried food debris. COS- moved to wash area. KDA_4_60111C - --There is mold and rust on the outside of the storage room chest freezer. There is food debris on the bottom shelf of the Amana chest freezer in the storage room. There is an accumulation of mold in the back room freezer that is not in use. There is an accumulation of grease in the oven, on the stove top, and on the grill in the kitchen. There is mold and rust on the rim of the kitchen chest freezer. KDA_4_90311A - --Located on the kitchen counter, there was a cup used to store clean utensils that had an accumulation of salt at the bottom of it. COS- cleaned out the cup. There is an accumulation of grease and food debris on the inside of the drawer used to store clean utensils under the counter of the kitchen island. Located on the kitchen microwave, there were two stacks of Styrofoam to go containers that were stored in a not inverted position. COS- inverted. KDA_6_50112A - --There is an accumulation of dust on the shelves and on the floor of the storage room and utility room. There is an accumulation of mold and debris on the floor under the three vat sink. There is debris on the floor of the kitchen and back room.
