- KDA_4_30111 - --The ambient air temperature of the back row of the MT 1 was 47.8F. COS- moved all potentially hazardous foods from MT 1 to WIC.
- KDA_4_60111A - --Located on the dish rack at the back of the kitchen, there was one plastic food container stored as clean that was soiled with dried food debris and one plastic food container stored as clean that was soiled with sticker residue. Corrected on site (COS)- moved to wash area.
- KDA_4_60111C - --The vent covers in the walk in freezer and WIC are soiled with dust.
- KDA_4_60211E4 - --There is an accumulation of mold and debris under the soda nozzles of the customer self serve soda fountain.
- KDA_5_20515B - --There is a leak from the ice machine from the customer self serve soda fountain. PIC said maintenance has already been called for this.
