KDA_3_20215 - --A can of fruit stored on the can rack had severe denting on the sides. PIC COS and pulled for return. KDA_3_50114A2 - --Brisket in the WIC that was cooked yesterday at around 3PM was at 48, 49, 47. PIC COS by VD. KDA_3_50118A1 - --Corned Beef Hash in the RIC was dated 12/14 and held past 7 days. PIC COS by VD. KDA_3_50118A2 - --Diced Ham, Diced Turkey and Diced Tomatoes in metal containers in the WIC were not dated. PIC said that they were prepared on 12/28. PIC COS by date marking. KDA_4_20211A2 - --Two plastic containers stored and stacked as clean on top of the RIFs had visible cracking and chips on the corner. PIC COS by discarding the containers. KDA_4_50111B - --MT door seal and RIF door seal were not intact and tight fitting. KDA_4_50112 - --Cutting board on the MT had scoring and scratching making it difficult to effectively clean and sanitize. PIC COS by removing, washing, sanitizing and rotating to smooth side. KDA_4_60111A - --A knife stacked and stored as clean had visible soiling. Container that utensils were stored in had food debris. PIC COS by moving both items to be washed, rinsed, and sanitized. KDA_4_60111C - --The door seals on RIFs in kitchen had accumulation of dust, dirt, or food residue. Non cooking surfaces of the grill had visible accumulation of dust, dirt, and food residue. Grill hood had visible food and grease accumulation. KDA_4_60212B - --Microwave oven in the kitchen had visible food debris on the sides and ceiling of interior. PIC indicated that microwaves should be cleaned daily. KDA_5_20315 - --Facility had a Pepsi soda machine installed on 09/2015. The carbonator device that did not have a visible dual check valve with an intermediate vent preceded by a screen and the machine was not marked with a sticker indicating the machine was protected. PIC contacted Pepsi to have machine checked and either corrected with dual check valve and intermediate vent or marked if devices are internally installed. KDA_6_10111A - --Facility had missing tiles and cracks on floors near floor drains that are not easily cleanable and smooth. KDA_6_20214 - --Employee toilet room did not have a self closing door.
