KDA_4_60111A - --In the prep area there are 2 containers holding clean utensils. There is a sandwich wrapper as a liner in each. There is dried food debris in the bottom of the containers. COS, utensils moved to be cleaned and sanitized and the liners replaced. On the shelf over the 3-vat sink there were 2 food storage containers stored as clean with date marking stickers and residue left on them. COS, VD per another violation.
KDA_4_60111C - --The fan covers in the WIC have an accumulation of dust.
