KDA_3_30212 - --On a shelf below a metal prep table there was a large food grade bin with white powder substance and was not labeled. COS-labeled the container flour.
KDA_3_50118A2 - --In the RIC (below the small MT), there was an open carton of half and half without a date. The person in charge said it was opened yesterday morning (past 24 hours). COS-date marked the item.
KDA_4_20211A2 - --On the utensil rack there was one large plastic food grade lid with a cracked edge (physical hazard). COS-discarded.
KDA_4_60111A - --In a container by the heat lamps, there were 5 metal pan holders with stickers left on the and stored clean. On the rack above the 3 compartment sink there was one (20 qt) plastic food grade container with a sticker left on the side and stored as clean. On the utensil rack there was one metal pot strainer and one metal tong with black rubber tips with dried food debris on them and stored clean. COS-moved all of the items to the ware wash area.
KDA_4_60111C - --There is dust on the fan blowers in the alcohol WIC and in the food WIC. There is duct tape being used to hold a sign on one of the microwaves and it is starting to fray (can contain bacteria).
KDA_6_30111 - --There is no soap in the womens restroom. COS-replaced the soap in the dispenser.
KDA_6_50111 - --The metal frame/base board by the oven is peeling and the wall is exposed. This is a good harboraging area for pests and not easy to keep clean. Ceiling tiles in the food prep area are peeling.
KDA_6_50112A - --There was trash on the floor behind the True RIC located at the bar. There was mold above the door frame inside of the WIC. COS-cleaned.

KDA_4_20211A2 - --On the utensil rack there was one (1/6) plastic food grade container with a broken edge. COS-discarded.
KDA_4_60111A - --The large can opener connected to a prep table had dried food debris on the blade and was used last Saturday (11/7/15). COS-cleaned.
KDA_4_60111C - --There is dust build up on the fan blowers in the walk in cooler.
KDA_6_20215A3 - --The south back exit door has a 1/4-1/2" gap along the bottom. The northeast front door has a 1/4" gap along the bottom. This could allow the entrance for pests.
KDA_6_50111 - --There are 3 broken floor tiles in the women's restroom. (Note: easy to clean and maintain). There are 2 ceiling tiles above the SunFire oven that are peeling.
