- KDA_2_40111 - --Observed an employee cups with out a lid stored on the kitchen MT. COS PIC removed cup and covered the top with a lid.
- KDA_3_30211A1b - --In the kitchen MT reach in cooler (RIC), raw chicken was stored on a wire shelf over RTE soup. COS Employee removed raw chicken, and placed on bottom shelf.
- KDA_3_50116A2 - --In the kitchen MT RIC, cut lettuce had a measured temperature of 50F. Employee said the lettuce was cut 9/1 in the morning. COS Employee was educated. Cut lettuce was discarded.
- KDA_3_50118A2 - --In the kitchen MT RIC, heavy whipping cream did not have a open or discard date. Employee said it was opened 8/31. COS Date was put the container. In the kitchen MT prep line, tomato relish (contains tomatoes) did not have a open or discard date. Employee said it was made 8/31. COS Date was put on the container.
- KDA_3_60311A - --There is no disclosure statement or reminder for the burgers that at cook to order on the menu.
- KDA_6_20215A3 - --The exit door by the office, there is a 1 and 1/2 inch gap along the bottom of the door.
- KDA_6_30112 - --The bar handwashing sink did not have paper towels. COS Paper towels were provided during inspection.
- KDA_6_501111A - --In the bar area, there were 150 to 200 mouse droppings in the lower cabinets and along the bar wall. Observed a live roach by the trash can in the bar area. Pest control last visit was 8/18. PIC said he called the contracted pest control and the company will be out today, 9/2.
