- 4-302.14 - --There are no test strips available for the chlorine sanitizer used in the dish machine.
- 4-102.11(B)(1) - --Located on the shelving for foods there was one "thank-U" bag with dried red peppers inside and directly touching the bag. COS. Peppers were placed in approved container. educated staff.
- KDA_3_10111 - --Located under the cooking preparation table, there was one case of rice flour with rodent feces on the bags and on the bottom of the box. Some of the bags were broken at the seals. COS. Items were VD. Located in the WIC, there was one case of romaine lettuce laying out directly on top of dirty shelving. COS. Lettuce was VD.
- KDA_3_30211A1a - --Located in the WIC, there was one pan of raw fish, one pan of raw shrimp, and 3 pans of raw chicken sitting on the shelf directly above RTE romaine lettuce. COS. Raw meats were placed on bottom shelving. Educated staff.
- KDA_3_30212 - --Located in the cooking area, there was one container of brown substance with no label. PIC said it was mixed seasonings. COS. Item was labeled.
- KDA_3_30412B - --Located inside the cornstarch and rice containers, there was a regular bowl with no handle inside and touching the foods. COS. Bowls were removed. Educated staff.
- KDA_3_30412F - --Located at the steamed rice area, there were 5 scoops sitting in water at a temperature of 44.9F. COS. Scoops were removed. Educated staff.
- KDA_3_30414B1 - --Located on the shelving for clean dishes, there was an accumulation of more than 10 soiled wiping cloths sitting on the shelf above the clean dishes. COS. Cloths were moved.
- KDA_3_30511A1 - --Located in the storeroom, there was one case of seaweed and two cases of soda syrup sitting directly on the floor. The employee preparing and cutting raw meat, had the bowl for the meat sitting directly on top of the trash can. COS. Bowl was moved. Educated staff.
- KDA_3_50116A2 - --The make table was holding PHF foods above 41F. Foods were raw chicken 44.4F
- KDA_4_50111B - --The WIF door is broken and warped and does not fit the frame correctly which is allowing ice build up inside the unit.
- KDA_4_60111A - --The table mounted can opener blade had sticky debris on the food surface. COS. Item was washed. The scoops in the flour container had brownish dried food debris on the food surface. COS. Scoops were washed. There were pans and bowls with dried food debris sitting on shelving for clean dishes. COS. Pans were washed. There were two containers with sticky tape residue sitting on the shelf for clean dishes. COS. Containers were placed at dish area.
- KDA_4_60111C - --The shelving units in the WIC and in the kitchen have a large accumulation of rust, grease, and dried food debris. The exterior and lid of the ice machine has an accumulation of sticky black residue. The vent hood area has a large accumulation of grease residue. The area around the mixer and noodle machine have a large accumulation of dried food debris.
- KDA_4_60211E4 - --The ice machine guard plate has visible black mold on the edges.
- KDA_5_501115 - --The area around and under the grease dumpster has a large accumulation of grease on the ground. PIC said he did not know why it was on the ground. COS. Employees were cleaning it during inspection with hot soapy water.
- KDA_6_20215A3 - --The front door and back door have gaps in the middle and bottom that are more than 1/4 inches.
- KDA_6_30112 - --Located at the wait station, the hand sink did not have a hand drying device. COS. PIC placed single use towels at the sink.
- KDA_6_30311A - --There are approximately 4 lights not working in the kitchen area above the preparation tables and cooking areas and storage areas and it is difficult to see in the room.
- KDA_6_30511B - --There was a personal chef hat sitting directly on top of a can of pineapple and 2 boxes of single use gloves which are used for food preparation.
- KDA_6_501111A - --Located under the dish machine and in the window by the dish machine, there were approximately 5 live crawling roaches.
- KDA_6_501112 - --Located under the equipment and in storage areas and in the entire kitchen area along the base boards, there is a large accumulation of dead roaches and other bugs.
- KDA_6_501114A - --There are 4 storage areas in the FE. All of them have clutter, unused and broken equipment and are disorganized.
- KDA_6_50112A - --The doors of the WIC, WIF, exits, and storage rooms have a large accumulation of black sticky debris. The walls and ceiling in the kitchen area have a large accumulation of grease debris. The floors of the kitchen have a large accumulation of grease and dead bugs. The WIC floor has a large accumulation of dried food debris.
- KDA_6_50112B - --The floor areas of the entire FE and all of the storage areas have a large accumulation of dead bugs and aged dried rodent feces. Located inside one case of rice flour, there were several dried aged rodent feces. COS. This case was VD.
- KDA_7_10211 - --Located under the dish machine, there was a 5 gallon bucket with no label and it was hooked to the dish machine. PIC was not sure what it was, however he thinks it is rinse aide. There was a bucket of soap and a bucket of sanitizer also sitting there.
- KDA_7_20111A - --Located on the cooking preparation table shelf, there was one spray bottle of degreaser sitting beside 1 case of dry noodles. COS. Chemical was moved.
