- KDA_3_50116A2 - --A container of egg whites was setting on food prep table at 66F. Employee said it had been out for approximately 1 hour. COS - placed in cooler. A bowl of sliced tomatoes was setting in plastic tub with melted ice/water around it. Tomatoes were 48F. Employee said they had been out of cooler for 3 hours. COS - water drained, additional ice added to tub.
- KDA_3_50118A2 - --In walk in cooler, RTE sliced ham, wrapped in plastic clear wrap, did not have consume by date. Employee said it was opened from original package and sliced on 12/13/15. COS - ham dated.
- KDA_4_60111A - --The counter mounted can opener had dried food debris on blade. Last used over 24 hours prior, per employee. COS - moved to warewashing for cleaning.
- KDA_4_60111C - --There is a build up of grease on inside of vent hoods. There is a build up of food debris along the floor edges of walk in cooler. There is a build up of dried food around the door seals of undermount cooler. There is a build up of lint on fan guards in walk in cooler.
- KDA_5_20511B - --The hand sink in kitchen had soiled metal whisk setting down in sink. Corrected on site (COS) - whisk removed from sink, educated.
- KDA_6_20211A - --The light in the walk in cooler does not have a shield over it.
- KDA_6_20215A3 - --The back door, by dish machine, has a 1/2 inch gap along the top and 1/4 inch gap along bottom of door.
- KDA_6_50111 - --There are several ceiling tiles in kitchen/dish area that have water damage. There are 2 floor tiles broken in kitchen area.
- KDA_6_501111A - --In food prep area, north of grill area, there were 2 live roaches crawling on food prep table. Establishment is serviced monthly by a pest control company.

Stove top - peppered gravy 168F. --In waitress station, wiping cloth sanitizer tested at 100ppm chlorine.
