  • KDA_3_10111 - --In the bar there is an opened bottle of Sauza Tequila that has three small flies floating inside. Corrected on Site (COS) - the general manager discarded this.
  • KDA_3_30212 - --At the grill there are unlabeled items that were identified by the general manager as salt, pepper, and pork seasoning. There are unlabeled tubs at the wall that he identified as breading for veggies and breading for chicken. There is an unlabeled shaker near the 3-section make table in the cooking area that he said was seasoning salt.
  • KDA_3_30412A - --There are single-use cups with no handles being used as scoops for the dijon mustard, homemade wing sauce, and homemade campfire shrimp sauce. These cups are stored directly in the containers in contact with the food in the container. These condiments are applied to the food product after is is cooked, immediately prior to serving.
  • KDA_3_50116A1 - --The right steam table near the grill has items that are not at proper hot holding temperatures. There are cooked mushrooms measuring 128.6 F and another pan of cooked mushrooms measuring 117.8 F. Per the general manager, these have been here since approximately 12pm. It was 210pm when the temperature was taken. COS - these were reheated.
  • KDA_3_50118A3 - --In the 3-section make table in the cooking area there is cut lettuce that is dated 1/18 - 1/25. There is homemade white sauce (for pasta) that is dated 1/18 - 1/25. There is cooked penne pasta that is dated 1/18 - 1/25. COS - the general manager corrected all these dates by taking them back one day. Explained that the initial date has to be counted as day one and that these are all dated for one day too many.
  • KDA_3_50119B2 - --In the make table where the salad items are stored
  • KDA_3_50119C2 - --At the breading station the chicken is dipped in a buttermilk mixture that is under time as a control. The time stamp and written procedures has it under a 6 hour hold time. The general manager said that he normally monitors the temperature of the mixture, but he was busy today due to the inventory and had not checked the temperature. COS - the temperature was checked and the mixture measured 66.7 F. Spoke with the general manager about making sure the temperature is always monitored, by him or someone else if he is unavailable.
  • KDA_4_20211A2 - --The crouton container has a crack along the edge of the container. COS - the general manager had this container changed out. There are two cracked containers and one chipped lid stored on a shelf in the dry storage room. COS - the general manager discarded these.
  • KDA_4_50111A - --There is peeling paint and rust buildup on the floor in the walk-in-cooler prep area.
  • KDA_4_60111A - --A container coming out of the dishwasher as clean has food residue caught near the corner inside of the container. COS - this was returned to dirty dishes by the general manager. There is a food container lid stored on a shelf as clean in the back room that has food residue caught on the outside of the lid. COS - the lid was taken to ware washing by the general manager. The vent hood has grease buildup that is dripping down on the edge of the grill below.
  • KDA_4_60111C - --The light covers in the vent hood have an accumulation of grease on them, as do the pipe lines coming down from the vent hood. There is food debris/crumbs caught in the door seal of the reach-in-freezer in the cooking area. There is debris on the interior base of this freezer. The 3-section make table in the cooking area has food debris caught on the ledge above the open food containers. There is an accumulation of dust on the fans in the walk-in-cooler. There is a container near the fans where the cover has blown off of the container.
  • KDA_4_90311C - --There are styrofoam to-go containers on the bottom shelf of a prep table in the waitressing area. They are outside of their original protective packaging and are neither covered nor inverted. The shelf above them has crumbs on them from fallen food debris. COS - the general manager had these inverted.
  • KDA_5_20212A - --The hand sink in the waitressing prep area is reaching 94.3 F after being left running for 5+ minutes. The hand sink in the baking prep area is reaching 93.2 F after being left running for several minutes. COS - A plumber arrived during the inspection and adjusted the water so that both sinks are providing 100 F and higher. Until the sinks were fixed, employees were able to use a hand sink in the kitchen that was reaching proper hand washing temperatures.
  • KDA_6_50112A - --There is food debris and dirt buildup below the ice machine.
  • KDA_7_10211 - --There is an unlabeled white tub at the ware washing machine with a cloudy liquid inside. The general manager said this was Silver Power cleaner for soaking the silverware in prior to washing. Let him know that this tub needs to be labeled to identify the chemical inside.
  • KDA_7_20111A - --In the cooking area there is a bucket of sanitizer stored directly next to stacks of clean plates and a container of seasoning. COS - the general manager moved the sanitizer.
