- KDA_3_10111 - --In reach in cooler, ROP packaged salmon, placed in cooler over 24 hours prior (per manager) was not vented. COS - salmon disposed of in trash.
- KDA_3_50118A1 - --In reach in cooler, a container of cooked rice had prep date of 11/24/15, held over 7 days. COS - rice disposed of in trash. In reach in cooler, 4 deli sandwiches (prepared on site) had prep date of 11/27/15, held for more than 7 days. COS - disposed of in trash.
- KDA_3_50118A2 - --In reach in cooler, a container of sliced tomatoes and container of cut lettuce (cut/sliced on 12/02/15, per manager) did not have prep date. COS - containers date marked. In walk in cooler, an opened gallon of milk did not have consume by date. Opened 12/02/15, per manager. COS - milk dated.
- KDA_6_20211A - --The light bulb in walk in freezer did not have protective shield.
- KDA_Defacto - --Cold Holding: Reach in cooler - cooked rice 39F. Refrigerator - cut lettuce 38F. Walk in cooler - twice baked potatoes 40F. Bar cooler - 41.6F ambient. Freezer ok. --Dish machine tested at 100ppm chlorine. NOTE: Sanitizer level would not show on test strip when testing dish machine in bar area. Not actively doing dishes. Manager will contact serviceman to look at it. --Hot Holding: Prep area - broccoli soup 145F.
