According to the Manager, they have been stored there for thirty minutes. Note: The temperature was taken at 1:35 p.m. Corrected on Site - Moved to Walk In Cooler immediately.
- KDA_3_50116A1 - --In the Steam Table, there was an egg roll at 117.7F. and an egg roll at 123.7F. According to the Manager, the egg rolls have been in the Steam Table for 1 hour. Corrected on Site - Manager Voluntary Discarded. Note: The manager was informed the egg rolls could be reheated.
- KDA_4_10111A - --In Maxx Cold Reach In Cooler, there were 2 non-food grade Sterilite Containers with fully cooked ready to eat chicken in direct contact with the container. Corrected on Site - Removed Chicken from the containers to stainless steel containers.
- KDA_4_60111A - --On the Drying Rack above the 3 Vat Sink, there were 2 black plastic containers (stored as clean) have dried food debris on the food contact surfaces. Corrected on Site - Moved to 3 Vat Sink. On the Drying Rack of the 3 Vat Sink, there were 2 stainless steel containers (stored as clean) with dried food debris on the food contact surface. Corrected on Site - Moved to 3 Vat Sink.
- KDA_4_60111C - --In the Walk In Cooler, there is dust build up on the fan blade covers, which is directly over the food items.
- KDA_4_60211E4 - --The Pepsi Soda Fountain has mold build up on the inside of the ice dispensing nozzle.
- KDA_4_90311B - --On the Drying Racks above the 3 Vat Sink, there were 4 (stored as clean) black plastic containers stacked together with water standing. Corrected on Site - Unstacked and allowed to air dry. On the Drying Shelf at the southend of the 3 Vat Sink, there were 2 stainless steel container stacked together with water standing on the containers. Corrected on Site - Unstacked and allowed to air dry.
- KDA_6_20215A3 - --The entrance doors to the facility have 1/2 inch gaps between the double doors, which is enough room for pest to enter the facility.
- KDA_6_50111 - --There is a 3 inch opening at the base of the wall behind the fryers, that is large enough for pest to enter into the facility. There is a 2 inch opening at the base of the wall behind the bulk ice machine, which is large enough for the pest to enter into the facility.
- KDA_6_501111A - --There are approximately 40 fresh rodent excreta on the floors throughout the northeast storage area. There are approximately 25 fresh rodent excreta on the floors throughout northwest storage room. There are approximately 60 fresh rodent excreta on the floors behind the fryers and bulk ice machine in the kitchen area. There are approximately 30 fresh rodent excreta underneath the Pepsi Soda Fountain in the cabinet below the machine. Note: The last pest control visit was on 1/20/16. The report stated there was no rodent evidence seen. There are 5 glue traps seen in the facility.
